A mortalidade infantil representa importante problema de saúde pública, para o estado de
Pernambuco, expresso pela persistência de coeficientes elevados se comparadas aos
coeficientes médios encontrados para o Brasil, regiões mais desenvolvidas do país e
entre países com renda per capita e Produto Interno Bruto (PIB) semelhante aos do
Brasil. Estes achados parecem estar correlacionados às condições de vida das
populações, sobretudo de grupos populacionais desprivilegiados aos quais faltam ou são
providos de forma escassa serviços essenciais. Apesar disto, o problema é
reconhecidamente pouco conhecido. O objetivo deste estudo foi descrever a distribuição espacial da mortalidade infantil, entre
1996-1998, segundo Condições de Vida, e considerando as Mesorregiões no Estado de
Pernambuco, com base numa estimativa baseada em princípios diferenciados dos até
então utilizados. Os coeficientes brutos de mortalidade por causas e faixas etárias foram calculados para
cada município para os anos de 1996-1998, pelo software SPSS for Windows versão 8.0
e corrigidos pelo modelo de Szwarcswald et al. Para 8 dos 177 municípios estudados não
foi possível obter a estimativa. Em outros casos, apesar de se ter conseguido a estimativa
do coeficiente de mortalidade, supõe-se ainda um razoável grau de subregistro. Foram construídos quintis de condições de vida para os 169 municípios do estado de
Pernambuco, para os quais foi possível estimar a mortalidade infantil, a partir dos quais
foram constituídos conglomerados homogêneos de municípios através de um indicador
sintético – o Índice de Condições de Sobrevivência Infantil (ICSI), constituído a partir dos
subcomponentes do ICV, hierarquizados segundo sua correlação com a mortalidade
infantil estimada (1996-1998): ICV-Educação, ICV-Infância, ICV-Renda e ICV-Habitação. Esses conglomerados foram descritos segundo seus Índices de Condições de
Sobrevivência Infantil, proporções da distribuição dos coeficientes de mortalidade infantil
estimados acima de 60/1000, entre 40-60/1000, entre 30-40 e abaixo de 30/1000
nascidos vivos, e também para as mesmas faixas de Coeficientes por Mesorregiões do
estado. Foram ainda calculadas as razões de taxas entre as proporções de nascimentos
“sem pré-natal” e de óbitos “sem assistência médica” para os conglomerados
homogêneos de municípios. Os resultados mostraram a existência de correlações entre os níveis de mortalidade
infantil e as condições de sobrevivência infantil, aí incluídas as questões de
disponibilidade de serviços de saúde.
Abstract
Infant Mortality is an important health problem of public health in the State of
Pernambuco, and it is showed by the maintenance of its high rates if compared to the
media rates found in Brasil and its more developed Regions, and among countries with
per capita income and Crude Internal Produce comparable to the one from Brasil. These
data seams to be correlated to the population Life Conditions, mainly to the some
population groups that are deprived or are provided with scarcity with essential services.
Though, the problem is least known.
The objective of this study is to describe the espatial distribution of the Infant Mortality in
the State of Pernambuco, for three years period from 1996 to1998, considering the
population life conditions , based in different principles to the one’s that have been used
before.
The crude rates of mortality by causes and age were calculated for each one of the
municipalities to the three years period of 1996-1998, using the software SPSS for
Windows version 8.0 and corrected by the Szwarcswald et al Model of Estimation . For 8
from the 177 municipalities studied it was not possible to obtain an estimation of the rates.
In other cases, though an estimation has been obtained, it is certainly correct to suppose
the existence of some degree of subestimation.
The 169 municipalities for which it was possible to estimate the infant mortality rate were
divided in 5(five) groups, based in their life conditions.
These groups were composed by municipalities with homogeneous life conditions based in
an sinthetic indicator – the index of the infant survival Conditions, which was constructed
using the subcomponents of the ICV, hierarchiezed by their correlation with the infant

mortality estimated (1996-1998): ICV-Educação, ICV-Infância, ICV-Renda e ICVHabitação.
The groups were then described by their indexes of the infant survival conditions,
correlated to the proportion of the distribution of the infant mortality rate estimated above
60/1000, between 40-60/1000, between 30-40 and below de 30/1000 live births, and for
the same indexes intervals for the Mesorregions of the State.
It was also calculated the ratio between the proportions of live births without any pre-natal
and the one’s between the proportions of infant death witout any medical assistance for
each one of the five groups.
The results showed the existence of correlation between the level of infant mortality of the
groups and the infant survival conditions, including the conditions more related to the
health services at disposal to the population in need.
