Objetivo: Avaliar a associação da síndrome metabólica (SM) com a atividade física e condições socioeconômicas entre idosos não institucionalizados. Metodologia: Estudo transversal com base nos dados do Estudo Saúde Bem-Estar e Envelhecimento realizado no ano de 2010. A população deste estudo foi selecionada de forma probabilística, sendo representativa de pessoas de 60 anos e mais, de ambos os sexos, não institucionalizadas e residentes na cidade de São Paulo. A variável dependente foi a síndrome metabólica classificada com base nos critérios adotados pela National Cholesterol Education Program Adult Treatment Panel III de 2001. As variáveis independentes foram sociodemográficas (sexo, idade, estado marital e escolaridade), comportamento em saúde (prática de atividade física) e condições de saúde (número de doenças crônicas não transmissíveis, sintomas depressivos e autoavaliação do estado de saúde). Para as análises foram utilizados o teste qui-quadrado com correção de Rao-Scott e regressão
logística múltipla com nível de significância de 5%. O efeito moderador da
atividade física na relação entre escolaridade e a SM foi testado por meio da inclusão do termo de interação entre a atividade física e a escolaridade. Com base nos resultados do modelo múltiplo foram calculadas a Fração atribuível (FA) e a Fração atribuível proporcional (FAP). A magnitude das desigualdades foi avaliada por meio do Índice Absoluto de Desigualdade (SII) e pelo Índice Relativo de Desigualdade (RII). Resultados: A prevalência de SM foi de 40,1% (IC 95% 37,1% - 43,2%), sendo que 23,3% (IC 95% 20,8% – 25,9%) dos idosos apresentavam pelo menos uma condição, sendo o aumento da pressão arterial sistólica o mais prevalente (66,1%; IC 95% 62,8% - 69,2%). A chance de SM foi significativamente menor entre os idosos mais velhos e entre aqueles fisicamente ativos. Idosos menos escolarizados apresentaram prevalências de SM significativamente maiores em termos absolutos e relativos [SII= -0,12 (IC 95% -0,231; -0,017) e RII= 0,73 (IC 95% 0,535 – 0,931)]. As FA e FAP entre os inativos e na população foram estatisticamente significantes, demostrando que a prática de atividade física pode reduzir significativamente a prevalência da SM na população (FA3,71%; FAP 9,27%) e especialmente entre os inativos (FA 8,75%; FAP 19,9%). Conclusão: Esse estudo demonstrou que a prática de atividade física e a escolaridade são fatores significativamente associados à SM. Esses achados demonstram a necessidade de se pensar em políticas que visem o envelhecimento saudável e a redução das desigualdades sociais por
meio da educação.
Abstract
Objective: To evaluate the association of metabolic syndrome (MetS) with
physical activity and socioeconomic conditions among non-nstitutionalized elderly. Methodology: A cross-sectional study based on data from the Estudo Saúde Bem-Estar e Envelhecimento conducted in 2010. The population of this study was selected in a probabilistic way, being representative of people of 60 years and over, of both sexes, non-institutionalized and residents of the city of São Paulo. The dependent variable was the MetS classified according to the criteria adopted by the 2001 National Cholesterol Education Program Adult Treatment Panel III. The independent variables were sociodemographic (sex, age, marital status and schooling), health behavior (physical activity practice) and health conditions (number of chronic noncommunicable diseases, depressive symptoms and self-assessment of health status). The chi-square test with Rao-Scott correction and multiple logistic regression with significance level of 5% were used for the analyzes. The moderating effect of physical activity on the relationship between schooling and MetS was tested through the inclusion of the interaction term between physical activity and schooling. Based on the results of the multiple model, the attributable fraction (AF) and the proportional attributable fraction (PAF) were calculated. The magnitude of the inequalities was evaluated through the Absolute Inequality Index (SII) and the Relative Inequality Index (RII). Results: The prevalence of MetS was 40.1% (CI 95% 37,1% – 43,2%), and 23.3% (CI 95% 20,8% - 25,9%) of the elderly had at least one condition, with an increase in systolic blood pressure the most prevalent (66.1%, 95% CI 62,8% - 69,2%). The chance of MetS was significantly lower among the elderly and among chose physically active. Less scholarized individuals had significantly higher prevalences of MetS in absolute and relative terms [SII = -0.12 (95% CI -0,231; -0,017) and RII = 0.73 (CI 95% 0,535-0,931)]. AF and PAF among the inactive and in the population were statistically significant, demonstrating that the practice of physical activity can significantly reduce the prevalence of MetS in the population (AF 3,71%, PAF 9,27%) and especially among the inactive (AF 8,75%, PAF 19,9%). Conclusion:
This study demonstrated that the practice of physical activity and schooling are factors significantly associated with MetS. These findings demonstrate the need to think about policies aimed at healthy aging and
reducing social inequalities through education.
