PLASMA FRESCO CONGELADO COMO INSUMO FARMACÊUTICO PARA A PRODUÇÃO DE HEMODERIVADOS NO BRASIL: UMA AÇÃO DE VIGILÂNCIA SANITÁRIA

Este trabalho foi fundamentado na análise das unidades de Plasma Fresco Congelado (PFC) quanto ao teor de Fator VIII recuperado como insumo farmacêutico para produção de hemoderivados. A fim de executar este objetivo foi necessária elaboração de questionário, o pré-teste para torná-lo validado e, aplicação em 72 visitas técnicas aos Serviços de Hemoterapia (SHs) do país. Concomitantemente, foram coletadas 05 (cinco) unidades de PFC que foram analisadas quanto ao teor de Fator VIII. Para análise dos indicadores do questionário foi utilizado a Tríade de Donabedian – Estrutura – Processos e Resultados e para os indicadores não informados (NI), este percentual foi omitido neste resumo. Para atender ao atributo Estrutura foram avaliados o tipo de serviço, formação de recursos humanos e equipamentos. Quanto ao atributo Processo foram avaliados a produtividade e sistema da qualidade e por fim o atributo Resultado foram avaliados o volume das unidades de PFC, tamanho do segmento da bolsa e análise do teor de Fator VIII recuperados nas unidades de PFC. Dos resultados obtidos: quanto a Estrutura- 33% correspondeu aos SHs (incluídos os privados, privados conveniados ao SUS e filantrópicos); 32% correspondeu aos Hemocentros Coordenadores (HC); 18% aos Hemocentros Regionais (HR); 10% aos Hemonúcleo (HN) e 7% as Unidades de Coleta e Transfusão (UCT). Ainda foram analisados quanto a natureza- 71% correspondeu aos serviços públicos; 21% aos privados conveniados ao SUS e filantrópicos e 8% aos serviços privados. Quanto a formação de recursos humanos- 57% correspondeu aos profissionais graduados e especializados; 40% aos de nível médio e apenas 3% aos profissionais graduados com mestrado ou doutorado. Quanto aos equipamentos existentes nos SHs: a) Geradores de Energia- 83% conta com este equipamento e 14% não; b) Sistema Informatizado- 83% possuem informatização dos cadastros dos doadores, 15% não; c) Equipamentos para Congelamento- 86% possuem e 8% não. Quanto aos processos de trabalho: a) Manutenção Preventiva- 82% possuem e 12% não; b) Calibração de Instrumentos- 60% possuem e 35% não; c) Auditorias Internas- 58% sim e 31%- não; d) Qualificação como Fornecedor de PFC para a HEMOBRÁS- 31%- sim e 62%- não, entre outros atributos. Quanto aos resultados foram avaliados: a) Produtividade- PFC- 93% e PC- 7%; b) Tempo de espera entre a coleta e o congelamento do PFC- até 6 horas- 85%; de 6 a 8 horas- 7%; de 18 a 24 horas- 3%; de 24 a 72 horas- 5%; c) Volume- média- 214,78mL, desvio padrão- 118,89mL, coeficiente de variação- 55,4%; d) Tamanho do Tubo Coletor- ≥ 15cm- 86% e ≤15 cm- 14%; e) Dosagem do teor Fator VIII no PFC- média- 0,097; desvio padrão- 0,168; valor p- 0,629 e valor F- 0,579. Foi aplicado o Teste ANOVA One Way demonstrando que não houve diferença significativa do teor de Fator VIII contido nas unidades de PFC produzido nos diferentes Serviços de Hemoterapia do país. Dentre as evidências e resultados obtidos, este trabalho conclui que as unidades de PFC produzidas nos serviços visitados, possuem especificação como insumo farmacêutico para a produção de hemoderivados no país, uma ação de Vigilância Sanitária.
Abstract
This work was based on the analysis of units of Fresh Frozen Plasma (FFP) and the level of factor VIII recovered as pharmaceutical ingredient for the production of blood products. It was necessary to elaborate a questionnaire to reach this goal (Appendix C), conducting a pre-test to validate it and finally apply it in 72 technical visits to Hematology Services throughout the country, and simultaneously collect five (05) FFC units were analyzed for the level of Factor VIII. To analyze the indicators of the questionnaire Donabedian Triad - Structure - Processes and Outcomes were used. Indicators not informed (NI), and blank gaps were omitted in this abstract. To meet the Structure attribute, the type of service, training of human resources and equipment were evaluated; for the Process attribute, productivity and quality system were evaluated; and finally, for the Results attribute the volume of PFC units, the segment size of blood bags and the analysis of the level of recovered factor VIII of FFC units were evaluated. Results: analysis of Structure results - 33% corresponded to blood banks (including private, private under contract to SUS and philanthropic); 32% corresponded to Coordinator blood centers; 18% to the regional blood centers; 10% to blood cores and 7% to Collection and Transfusion Units. They were also analyzed by nature - 71% corresponded to public services; 21% corresponded to private under contract to SUS and philanthropic and 8% to private services. As for training of human resources, 57% corresponded to graduated and post graduated professionals; 40% to high school level professionals and only 3% to masters or doctors. As for the existing equipment in SHs: a) Energy Generators - 83% have this equipment and 14% do not have; b) Computerized System - 83% do have for donors registration and 15% do not have; c) Freezing Equipment - 86% do have and 8% do not have, among other equipments. As for work processes: a) Preventive Maintenance - 82% do have and 12% do not have; b) Calibration Instruments - 60% do have and 35% do not have; c) Implementation of Internal Audits - 58% yes and 31% no; d) Qualifications as a supplier of PFC to HEMOBRÁS - 31% yes and 62% no, among other attributes. About the results, were evaluated: a) productivity: FFC - 93% and PC - 7%; b) Waiting time between collection and FFC freezing - up to 6 hours - 85%; 6-8 hours 7%; 18-24 hours - 3%; 24-72 hour - 5%; c) volume – average of 214,78mL, standard deviation of 118,89mL, variation coefficient of 55.4%; d) Tube collector size - ≥15cm - 86% and ≤15cm - 14%; e) dosage of factor VIII level in FFC – average - 0.097; standard deviation - 0.168; value P - 0.629; value F - 0,579. All results show that there is no difference between the analyzed groups. Among the evidence and results presented, the conclusion of this work is that the units of produced FFC in those 72 visited services throughout the country also has specification as pharmaceutical ingredient for the production of blood products in the country, an action for Sanitary Surveillance.
