O Programa Médico de Família (PMF) traduz-se como uma política voltada para a
organização da atenção primária à saúde em Niterói desde 1992. Durante os 22 anos de
sua implementação, o programa caracterizou-se por um conjunto de ações de saúde
centrado no cuidado à família e realizado por uma equipe, composta por médico e técnico
de enfermagem. Entretanto, a partir de 2006, percebem-se algumas mudanças na
composição das equipes e cobertura do PMF. Neste estudo, entende-se que além dos
incentivos federais, variáveis locais podem ter favorecido alterações do PMF e sua
aproximação ao modelo preconizado em nível nacional. O objetivo da pesquisa foi
analisar a trajetória da implementação do PMF em Niterói com ênfase nos elementos de
continuidades e mudanças nos anos 2000. Foi realizado um estudo de caso com
abordagem qualitativa. As técnicas de pesquisa empregadas foram a análise documental e
entrevistas semiestruturadas. Os sujeitos pesquisados foram: secretários municipais de
saúde, coordenadores do programa, supervisores e médicos das equipes, presidentes das
Associações de Moradores e docente ligado à implementação do programa. As
entrevistas foram realizadas no período entre maio a outubro de 2014. Utilizou-se a
análise de conteúdo cuja categorização foi baseada em duas dimensões: o contexto e a
institucionalidade do PMF. A análise da trajetória de implementação do PMF permite
identificar continuidades e mudanças em vários aspectos organizacionais e operacionais
do Programa. O foco da política continua sendo o atendimento à parcela da população
desprovida de serviços básicos, expandindo-se a cobertura das equipes que ampliaram
sua composição, o que suscita questionamentos quanto à manutenção do seu modelo
original. Destacam-se como fatores explicativos da continuidade do PMF nos anos 2000 a
trajetória histórica da saúde pública no município, o valor simbólico do Programa Médico
de Família para a cidade e a criação de uma rede de atores governamentais e não
governamentais que sustentam a política no município. Entre os fatores que explicam as
mudanças ressaltam-se as alterações na equipe de condução do Programa e nas condições
socioeconômicas e do mercado de trabalho na cidade e seu entorno, o alinhamento às
proposições do MS e os incentivos federais. Pode-se concluir que a compreensão desses
fatores contribui também para o entendimento da rede de saúde do município.
Abstract
The Family’s Doctor Program (FDP) is a policy toward the organization of Primary
Health Care in Niterói since 1992. During the 22 years of its implementation, the
program was characterized by a set of health actions focused on family and care by a
team, composed of doctor and nursing technician. However, since 2006 some changes are
perceived in the composition of the teams and on program coverage. Besides the federal
incentives, it is believed that local variables may have contributed for an approximation
of FDP to the model recommended in national level. The objective of the research was to
analyze the trajectory of the implementation of FDP in Niterói with emphasis on elements
of continuities and changes in the 2000s. It was realized an exploratory case study with
qualitative approach. The research techniques used were the documentary analysis and
semi-structured interviews. The subjects studied were: municipal managers of health,
program coordinators, supervisors and doctors, presidents of associations of residents and
professor involved in the program implementation. The interviews were realized in the
period between May to October 2014. For analysis of empirical findings were used the
thematic content analysis whose categorization was based on two dimensions: the context
and the institutional framework of the FDP. The analysis of the trajectory of
implementation of FDP allows identify continuities and changes in various organizational
and operational aspects of the program. The policy’s focus remains being the care to
portion of the population that lacks basic services, expanding the coverage of the teams
that have extended their composition, which raises some questions about the maintenance
of your original model. As explanations for the continuity of the program in the 2000s, it
were standing out the historical trajectory of public health in the city, the symbolic value
of the program for the city and the creation of a network of governmental and non
governmental organizations that support the policy in the municipality. Among the
factors that explain the changes, it were emphasized the change in team co-ordination of
the program, the socioeconomic conditions, the labor market in the city and its
surroundings, the alignment of the health ministry’s prepositions and the federal
incentives. It can be concluded that the understanding of these factors can also contribute
to the comprehension of the municipality’s health network.
