Introdução: A leishmaniose tegumentar é endêmica nas regiões Norte/Nordeste do Brasil e apresenta como formas clínicas a leishmaniose cutânea localizada (LCL), a muco-cutânea e a cutânea difusa. A leishmaniose cutânea localizada caracteriza-se pela formação de úlceras cutâneas úmidas, de cura difícil. Há formas distintas de apresentação: lesões mínimas, secas, vegetantes e disseminadas atípicas. Estas diferentes formas clínicas da LCL têm um perfil histológico distinto e quantidade parasitária variável, sendo desconhecidos fatores preditivos de evolução clínica e prognóstico. Objetivo: Avaliar o significado preditivo, no que se refere a evolução clínica, desenvolvimento da forma mucosa, espécie do agente etiológico e resposta a quimioterapia com antimoniais, de um conjunto de alterações histológicas e da carga parasitária em biopsias de pele de pacientes com diferentes apresentações clínicas de leishmaniose cutânea. Casuística e Métodos: Foram realizadas avaliações histológicas de biópsias de lesões, sem conhecimento dos dados clínicos-laboratoriais, observando os seguintes parâmetros: intensidade do parasitismo, perfil fenotípico das células do infiltrado inflamatório, granulomas, necrose, dermólise e apoptose. Resultados Preliminares: Foi construída uma base de dados constando de 458 pacientes com avaliação histológica. Foram identificados parasitos na lesão em 42% (192 casos). Destes 52,6% tinham granulomas. A identificação de parasitos pelo HE ocorreu em 40 casos, mas com o uso de imunoistoquímica outros 152 foram identificados. Os casos que não tinham parasitos na lesão apresentavam dermatite crônica (205 casos). Destes, 28,19% tinham granulomas. Conclusões: Construída a base de dados com os aspectos clínicos das diferentes formas de LCL tabulados, as biópsias serão avaliadas quanto aos demais aspectos histológicos tais como necrose, apoptose e dermólise e quanto ao seu potencial de resposta a quimioterapia com antimoniais e aspectos preditivos de evolução clínica com desenvolvimento de cura ou forma cutânea-mucosa.
