Este trabalho aborda a temática da eqüidade em saúde, mais especificamente, na distribuição geográfica dos recursos financeiros. Teve por objetivo avaliar a distribuição entre Unidades Federadas dos recursos federais destinados ao custeio dos serviços de saúde e contribuir para
a operacionalização de formas distributivas que visem à obtenção de patamares mais eqüitativos. Neste sentido, foi possível identificar que os mecanismos distributivos implementados pelo Ministério da Saúde na década de 90 não foram coerentes com o conceito de eqüidade preconizado para a política de saúde no texto constitucional, bem como que a
distribuição efetuada em 1994 continuou caraterizada pela falta de eqüidade e que a alocação deveria estar norteada pela ‘igualdade de recursos para necessidades iguais’ com o objetivo da obtenção de partilhas mais eqüitativas. Com essa finalidade, com base na discussão concernente às experiências internacionais, foi identificada como metodologia mais adequada a fórmula RAWP implementada na Inglaterra, que dimensiona as necessidades relativas a partir dos perfis tanto demográfico quanto
epidemiológico. O resultado obtido na simulação efetuada para o caso brasileiro mostrou que a aplicação da proposta inglesa, utilizando como fonte de informação a mortalidade notificada para dimensionar as desigualdades entre os perfis epidemiológicos das diferentes UFs,
definiria alocações semelhantes às realizadas pelo MS. Por sua vez, a distribuição obtida na simulação efetuada com base na mortalidade proporcional evidenciou que a partilha não se diferenciaria da estimada exclusivamente em base populacional. Estes resultados foram
determinados pelo alto nível de sub-registro nos óbitos notificados, cuja maior magnitude observa-se nos estados das regiões Norte e Nordeste.
Conseqüentemente, realizaram-se adaptações da metodologia RAWP, utilizando-se, por um lado, a mortalidade corrigida segundo nível de sub-registro e, por outro, indicadores sócioeconômicos associados às condições de vida – renda per capita e percentual de domicílios com renda inferior a um salário mínimo. Os resultados obtidos permitiram verificar que, em
todos os casos, para uma distribuição geográfica de recursos financeiros mais eqüitativa, seria necessário pôr em execução uma redistribuição em favor dos estados das regiões Norte e Nordeste. Ao mesmo tempo, tornou-se possível constatar especificamente que o tratamento
menos eqüitativo tem-se dado entre os estados da região Nordeste e os correspondentes à região Sudeste, em favor destes últimos.
Abstract
This work approaches the theme of the equity in health, specifically, equity in the geographical distribution of the financial resources. The objectives are to evaluate the federal resources distribution destined to the costing of the services of health between Federated Units (UF) and to contribute for the construction of distributive tools more equal. The distributive mechanisms implemented by Ministry of Health (MS) in 90’showed incoherent
with the concept of justness express for the health policy in the constitutional text, which it referred although the allocation should be orientated by the ‘‘ equality of resources for same needs ‘‘with the objective of obtaining a more equal distribution. In 1994, the distribution of resources continued being characterized by the lack of justness. With these objectives, considering the international experiences was identified as more appropriate methodology the formula RAWP implemented in England. This methodology measure the relative needs using demographic and epidemic profile. This proposal was tested for the Brazilian case. The database used was the Mortality Information System (MIS), based on the mortality causes information. The results showed that the English proposal’s application for measure inequalities among the differences in epidemiological profiles of the
Federated Units would define resources allocations quite similar to accomplished for MS. The results of estimates with population and proportional based were also compared. In this comparison was observed that the resources distributions obtained were similar. High subregistration level in the notified deaths can explains these results, whose larger magnitude observed was in North and Northeast states. Consequently, adaptations in the methodology RAWP were done, using the following
variables: (i) the mortality rate corrected by sub-registration level; (ii) the socioeconomic conditions indicators (per capita income and percentile of households with minimum income). The results obtained in this work allowed to verify, in all analyses accomplished that, for a larger equity in geographical resources distribution would be necessary to redistribute resources in favor of North and Northeast States. Particularly, it was still possible to verify
that unequal distribution of resources has been observed among the Northeast and Southeast States, in favor of these last ones.
