As leveduras do gênero Candida são fungos oportunistas que podem ser encontrados como comensais em diversos sítios do corpo humano e no ambiente. Quando há o desequilíbrio da microbiota humana, essas leveduras tornam-se patógenos e podem desenvolver candidíase, uma infecção fúngica que afeta tanto indivíduos saudáveis quanto imunocomprometidos. O presente estudo teve como objetivo verificar e comparar os fatores de virulência de 48 leveduras isoladas do ambiente aquático e da mucosa oral de idosos da comunidade rural Lago do Limão, Iranduba-Am, bem como avaliar sua susceptibilidade aos antifúngicos itraconazol, anfotericina B, cetoconazol e fluconazol. Foram realizados testes de produção de protease, hemolisina, fosfolipase e urease, com os meios de cultura Ágar Proteinase, Ágar Sabouraud acrescido de sangue de carneiro, Ágar Sabouraud suplementado com gema de ovo e Ágar Ureia de Christensen, respectivamente. Para o teste de sensibilidade a antifúngicos, foi utilizado o método Etest e a análise estatística foi realizada com os testes Exato de Fisher e Qui-quadrado. Observou-se a prevalência do gênero Candida, com destaque às espécies não-albicans. Foram identificadas 45 leveduras: C. albicans (6,6%), C. glabrata (33,3%), C. parapsilosis (24,4%), C. tropicalis (8,8%), Meyerozyma guilliermondii (13,3%), Meyerozyma caribbica (4,4%), C. metapsilosis (2,2%), Saccharomyces cerevisiae (2,2%), Kodamaea ohmeri (2,2%) e Lodderomyces elongisporus (2,2%). 68,7% das leveduras tiveram atividade hemolítica positiva, 47,9% produziram proteases, 16,6% produziram fosfolipases e todas foram urease negativas. Não houve diferença estatística significativa entre a virulência de leveduras de origem clínica e ambiental. Todas as leveduras foram sensíveis ao antifúngico anfotericina B; 37,5% foram sensíveis ou sensíveis dose dependentes (SDD) ao cetoconazol, 35,4% sensíveis ou SDD a fluconazol e 29,1% sensíveis ou SDD a itraconazol, com concentrações inibitórias mínimas (MICs) de 0,094 ~ 1 µg/ml; 0,094 ~ 16 µg/ml; 1,5 ~ 128 µg/ml e 0,125 ~ 8 µg/ml, respectivamente. Dessa forma, com o conhecimento dos processos infecciosos e de como as enzimas atuam, associado à constante busca pelo melhor medicamento, há contribuição para a criação de novas estratégias de prevenção e tratamento para candidíase, melhorando assim a qualidade de vida das populações mais vulneráveis na região - os ribeirinhos.
Abstract
Candida yeasts are opportunistic fungi that can be found as commensals in several places in the human body and in the environment. When human microbiota imbalance occurs, these yeasts become pathogens and can develop candidiasis, a fungal infection that affects both healthy and immunocompromised individuals. The present study aimed to verify and compare the virulence factors of 48 yeasts isolated from the aquatic environment and from the oral mucosa of elderly people from the rural community Lago do Limão, Iranduba-Am, as well as to evaluate their susceptibility to the antifungals itraconazole, amphotericin B, ketoconazole and fluconazole. Protease, hemolysin, phospholipase and urease production tests were carried out with the culture media Agar Proteinase, Sabouraud Agar plus sheep's blood, Sabouraud Agar supplemented with egg yolk and Christensen's Urea Agar, respectively. For the antifungal sensitivity test, the Etest method was used and the statistical analysis was performed using Fisher's Exact and Chi-square tests. A higher prevalence of the genus Candida was obtained, with emphasis on non-albicans species. Forty-five yeasts were identified: C. albicans (6.6%), C. glabrata (33.3%), C. parapsilosis (24.4%), C. tropicalis (8.8%), M. guilliermondii (13 .3%), M. caribbica (4.4%), C. metapsilosis (2.2%), S. cerevisiae (2.2%), K. ohmeri (2.2%) and L. elongisporus (2,2%). 68.7% of the yeasts had positive hemolytic activity, 47.9% produced protease, 16.6% produced phospholipase and all yeasts were urease negative. There was no statistically significant difference between the virulence of yeasts of clinical and environmental origin. All yeasts were sensitive to the antifungal amphotericin B; 37,5% were sensitive or sensitive dose-dependent (SDD) to ketoconazole, 35,4% were sensitive or SDD to fluconazole, and 29.1% were sensitive or SDD to itraconazole, with minimum inhibitory concentrations (MICs) of 0.094 ~ 1 µg/ml ; 0.094 ~ 16 µg/ml; 1.5 ~ 128 µg/ml and 0.125 ~ 8 µg/ml, respectively. Thus, with the knowledge of infectious processes and how enzymes act, associated with the constant search for the best drug, there is a contribution to the creation of new prevention and treatment strategies for candidiasis, thus improving the quality of life of the most vulnerable populations in the region - the riverside people.
