Os vírus entéricos são causas importantes causas de gastroenterite aguda e a vigilância é fundamental para entender a dinâmica da infecção e prevenir novos casos. RV, NoV, AdV e AstV são os patógenos mais encontrados em surtos. Aqui, analisamos os aspectos clínico-epidemiológicos de pacientes infectados por esses vírus, bem como o perfil molecular do NoV no estado de Pernambuco. O estudo foi composto por pacientes com gastroenterite aguda e suas amostras foram encaminhadas ao LACEN/PE para diagnóstico. Informações de variáveis clínicas e epidemiológicas foram obtidas através de questionário aplicado por profissionais de saúde e as amostras foram testadas por ensaio imunoenzimático (EIA). A identificação molecular foi realizada nas amostras positivas para NoV através de sequenciamento. De 2014 a 2017, 1.660 amostras foram coletadas em todo o Estado. A maioria foi oriunda de Recife (40.90%), seguido por Jaboatão dos Guararapes (10.06%) e Goiana (8.43%). Das 1.660 amostras, 448 (26.99%) foram positivas para pelo menos um patógeno. RV foi o mais detectado (57.81%), seguido por NoV (27.90%), AdV (10.27%) e AstV (4,02%). 18 amostras (4.02%) foram de infecções mistas, principalmente por RV e NoV. 229 indivíduos (51,11%) foram do sexo masculino e 219 (48,89%) do feminino. O grupo mais afetado foi crianças de 1 a 5 anos (40.84%) e menores de 1 ano (35.26%). Os sintomas mais prevalentes foram diarreia, fezes não sanguinolentas, vômito e febre. Nenhum padrão de sazonalidade foi observado. O norovírus GII.4 foi o genogrupo predominante e o genótipo mais prevalente foi o GII.Pe-GII.4, detectado em 16 (80.00%) amostras. Outros três genótipos também foram encontrados: GII.P16-GII.4 (10.00%), GII.P16-GII.3 (5.00%) e GII.P4-GII.4 (5.00%). Nosso estudo forneceu informações importantes sobre as características epidemiológicas dos vírus entéricos e o perfil molecular do norovírus no estado de Pernambuco.
Abstract
Enteric viruses are major causes of acute gastroenteritis, and surveillance is critical to understanding the dynamics of infection and preventing new cases. RV, NoV, AdV and AstV are the most common pathogens found in outbreaks. Here, we analyze the clinical and epidemiological aspects of patients infected by these viruses, as well as the molecular profile of NoV in the state of Pernambuco. The study consisted of patients with acute gastroenteritis and their samples were sent to LACEN/PE for diagnosis. Information on clinical and epidemiological variables was obtained through a questionnaire applied by health professionals and the samples were tested by enzyme-linked immunosorbent assay (EIA). Molecular identification was performed on samples positive for NoV through sequencing. From 2014 to 2017, 1,660 samples were collected across the state. Most came from Recife (40.90%), followed by Jaboatão dos Guararapes (10.06%) and Goiana (8.43%). Of the 1,660 samples, 448 (26.99%) were positive for at least one pathogen. RV was the most detected (57.81%), followed by NoV (27.90%), AdV (10.27%) and AstV (4.02%). 18 samples (4.02%) were from mixed infections, mainly by RV and NoV. 229 individuals (51.11%) were male and 219 (48.89%) were female. The most affected group were children aged 1 to 5 years (40.84%) and children under 1 year (35.26%). The most prevalent symptoms were diarrhea, non-bloody stools, vomiting and fever. No seasonality pattern was observed. Norovirus GII.4 was the predominant genogroup and the most prevalent genotype was GII.Pe-GII.4, detected in 16 (80.00%) samples. Three other genotypes were also found: GII.P16-GII.4 (10.00%), GII.P16-GII.3 (5.00%) and GII.P4-GII.4 (5.00%). Our study provided important information about the epidemiological characteristics of enteric viruses and the molecular profile of norovirus in the state of Pernambuco.
