A atual pandemia de COVID-19 representa uma crise aguda de saúde e economia global, com consequências alarmantes na mortalidade e morbidade humana. Diante disso, há uma necessidade urgente de maiores estudos acerca da resposta imune inata quanto adquirida frente a infecção pelo SARS-CoV-2, para poder tanto aumentar sua eficácia, quanto prevenir seus efeitos deletérios. Com isso, este trabalho teve como objetivo esclarecer a cronologia de anticorpos IgM/IgG, a cinética de mediadores solúveis séricos após COVID-19 e a aplicabilidade do uso dos testes rápidos indiretos disponíveis no Brasil. Para avaliar a resposta imune inata e humoral frente a infecção por SARS-CoV-2, amostras de 330 pacientes internados no Hospital da Baleia, Belo Horizonte, positivos e negativos para COVID-19, com amostras coletadas no início da internação até seis meses do aparecimento dos primeiros sintomas, foram coletadas para a determinação da resposta imune utilizando ELISA in house e citometria de fluxo, para posterior avaliação da utilidade e confiabilidade dos testes rápidos indiretos disponíveis para uso no Brasil. De um modo geral, os pacientes RT-qPCR+ passaram a ter IgM detectáveis por ELISA a partir da primeira semana de acompanhamento, o qual permaneceu detectável por até oito semanas. O anticorpo do tipo IgG apresentou altos títulos a partir da segunda semana do início de sintomas e permaneceu detectável durante os seis meses do estudo. Ao avaliar separadamente a cronologia dos anticorpos IgM e IgG entre os pacientes RT-qPCR positivos por idade, gênero, presença ou não de neoplasia e por gravidade clínica da doença, foi observado perfis de anticorpos semelhantes entre estes grupos, não tendo diferença estatística significativa. Ao realizar a quantificação dos biomarcadores solúveis sistêmicos, as quimiocinas, citocinas próinflamatórias, citocinas reguladoras e fatores de crescimento avaliados, apresentaram uma queda durante os seis meses de acompanhamento com exceção para CCL11, CXCL8, CCL3, CCL4, CCL5, IL-6, IFN-g, IL-17, IL-5, FGF-basic, PDGF, VEGF, G-CSF e GM-CSF. Após análise de anticorpos neutralizantes pelo teste ECO F COVID nAb (ECO Diagnóstica), um número significativo dos pacientes infectados apresentou anticorpos neutralizantes para a cepa de referência de Wuhan e para as variantes Alpha, Gamma e Beta. Ainda, ao avaliar os testes rápidos imunocromatográficos indiretos disponíveis no Brasil, os mesmos apresentaram uma boa sensibilidade e especificidade na detecção de anticorpos anti-SARS-CoV-2 após 15 dias de sintomas.
Abstract
The current COVID-19 pandemic represents an acute global health and economic crisis, with alarming consequences for human mortality and morbidity. Therefore, there is an urgent need for further studies on the innate and acquired immune response to SARS-CoV-2 infection, to both increase its effectiveness and prevent its deleterious effects. Thus, this study aimed to clarify the chronology of IgM/IgG antibodies, the kinetics of serum soluble mediators after COVID-19 and the applicability of the use of indirect rapid tests available in Brazil. To evaluate the innate and humoral immune response to SARS-CoV-2 infection, samples from 330 patients hospitalized at Baleia Hospital, Belo Horizonte, positive and negative for COVID-19, with samples collected at the beginning of hospitalization up to six months after admission. Appearance of the first symptoms, were collected for the determination of the immune response using in-house ELISA and flow cytometry, for later evaluation of the usefulness and reliability of the indirect rapid tests available for use in Brazil. Overall, RT-qPCR+ patients had ELISA detectable IgM from the first week of follow-up, which remained detectable for up to eight weeks. The IgG-type antibody showed high titers from the second week of symptom onset and remained detectable during the six months of the study. When separately evaluating the chronology of IgM and IgG antibodies among RT-qPCR positive patients by age, gender, presence, or absence of neoplasia and by clinical severity of the disease, similar antibody profiles were observed between these groups, with no statistically significant difference. When performing the quantification of soluble systemic biomarkers, the chemokines, pro-inflammatory cytokines, regulatory cytokines, and growth factors evaluated showed a decrease during the six months of follow-up, except for CCL11, CXCL8, CCL3, CCL4, CCL5, IL-6, IFN-g, IL-17, IL-5, FGF-basic, PDGF, VEGF, G-CSF e GM-CSF. After analysis of neutralizing antibodies by the ECO F COVID nAb test (ECO Diagnóstica), a significant number of infected patients had neutralizing antibodies to the Wuhan reference strain and to the Alpha, Gamma and Beta variants. Also, when evaluating the rapid indirect immunochromatographic tests available in Brazil, they showed good sensitivity and specificity in the detection of anti-SARS-CoV-2 antibodies after 15 days of symptoms.
