Abstract in Portuguese | O estudo, de caráter exploratório, longitudinal, retrospectivo e descritivo, objetivou analisar a contratualização, o perfil e o desempenho dos Hospitais Universitários Federais (HUF) geridos pela Empresa Brasileira de Serviços Hospitalares (EBSERH), antes e após a contratualização. Os perfis de complexidade da estrutura e das internações realizadas foram considerados e, para caracterizar o perfil de complexidade dos hospitais quanto ao consumo de recursos, foi elaborada uma medida de proxy para definir o case mix, como uma aproximação da complexidade dos HUF, utilizando os valores atribuídos pelo SUS para pagamento dos serviços hospitalares. Foram incluídos no estudo todos os HUF cuja contratualização se deu a partir de 2013 e se manteve até 2019. Realizou-se análise documental dos instrumentos contratuais firmados entre a EBSERH e os HUF – contratos, termos aditivos, e os anexos, descrição e classificação dos hospitais quanto ao perfil de complexidade da estrutura e do processo de atenção (case mix) de suas internações no momento anterior e posterior a gestão EBSERH; a evolução de indicadores de desempenho hospitalar selecionados também foi analisada. A caracterização dos hospitais considerou os dados referentes a número de leitos e de salas cirúrgicas, habilitações, complexidade e tipo de atendimento predominante para classificação em portes de I a IV agrupados nos perfis: Hospitais Especializados, Maternidades, Hospitais Gerais com e sem Urgência e Emergência. Para o desempenho selecionaram-se indicadores de estrutura, processo e resultados. A análise matemática e estatística dos dados secundários do Sistema de Informações Hospitalares-SIH e do Cadastro Nacional de Estabelecimento de Saúde – CNES, foi realizada com auxílio do software R. Na análise dos resultados optou-se por agrupar os HUF segundo seu perfil assistencial e porte. A análise dos instrumentos contratuais evidenciou contratos com prazo de vigência indeterminado (25%), 80% de unidades definidas como hospitais gerais, 70% destes como de médio e grande porte. No desempenho, não houve aumento significativo de estrutura em número de leitos, diferentemente do expressivo aumento de profissionais médicos (118%). Ampliou-se a média de complexidade em todos os perfis e portes, principalmente nas maternidades (24,93%). O tempo de permanência foi significativamente reduzido em todos os hospitais nas internações de baixa complexidade, enquanto as taxas de mortalidade hospitalar reduziram nas internações de média e muito alta complexidade nos hospitais de porte III. Conclui-se sobre a necessidade de aprimoramento da relação contratual, especialmente na diversificação de indicadores em acordo com os perfis de cada unidade. De modo geral, houve evolução de porte e perfil, aumento do número de profissionais e internações e redução de tempos de permanência e taxas de mortalidade hospitalar. | en_US |

Abstract | This exploratory, longitudinal, retrospective and descriptive study aimed to analyze the contracting, profile and performance of the Federal University Hospitals (HUF) managed by the Brazilian Company for Hospital Services (EBSERH), before and after contracting. The complexity profiles of the structure and the hospitalizations performed were considered and, to characterize the complexity profile of the hospitals regarding the consumption of resources, a proxy measure was developed to define the case mix as an approximation of the complexity of the HUF, using the values assigned by the SUS for payment of hospital services. All HUFs whose contractualization took place from 2013 and remained until. A documental analysis of the contractual instruments signed between EBSERH and the HUFs - contracts, additive terms, and the annexes, description and classification of the hospitals regarding the complexity profile of the structure and the care process (case mix) of their admissions at the time before and after the EBSERH management; the evolution of selected hospital performance indicators was also analyzed. The characterization of the hospitals considered the number of beds and operating rooms, qualifications, complexity and predominant type of care for classification into sizes from I to IV grouped in the profiles: Specialized Hospitals, Maternities, General Hospitals with and without Urgency and Emergency. For performance, indicators were selected: structure, process and results. The mathematical and statistical analysis of secondary data from the Hospital Information System-SIH and the National Health Establishment Registry - CNES was performed with the help of R software. In the analysis of the results, was chose to group the HUFs according to their care profile and size and use descriptive statistics as parameters for comparison between groups of hospitals. The analysis of the contractual instruments showed contracts with undetermined term (25%), 80% of units defined as general hospitals, 70% as medium and large hospitals. In performance, there was no significant increase in the structure in number of beds, unlike the significant increase in the number of medical professionals (118%). The average complexity was increased in all profiles and sizes, especially in maternity hospitals (24.93%). The length of stay was significantly reduced in all hospitals in low complexity hospitalizations, while hospital mortality rates were reduced in medium and very high complexity hospitalizations in size III hospitals. It is concluded that there is a need to improve the contractual relationship, especially in the diversification of indicators according to the profiles of each unit. In general, there was an evolution in size and profile, increase in the number of professionals and admissions and reduction in length of stay and hospital mortality rates. | en_US |
