A Leucemia Linfoide Aguda (LLA) de células T é uma neoplasia maligna hematológica
agressiva que representa 15% das LLAs da infância. Atualmente, são descritas
anormalidades genéticas específicas com importância no diagnóstico e prognóstico
da doença. Através de estudos citogenéticos e moleculares é possível identificar
oncogenes envolvidos na patogenia da doença. O objetivo do trabalho foi determinar
a frequência e o impacto das alterações nos genes FLT3 e Notch1 em pacientes
pediátricos com Leucemia de células T em Pernambuco e estabelecer seu perfil
genético. Foram estudados 129 pacientes com LLA-T no IMIP e no HUOC. A
citogenética foi avaliada por banda G e FISH, o perfil genético por PCR com o painel
estabelecido pelo Laboratório de Biologia Molecular do IMIP, e as alterações FLT3-
ITD e mutação na região PEST do Notch1 por sequenciamento. Em relação ao painel
molecular, dos 110 pacientes analisados, 14 foram positivos para a expressão do
gene TLX3 e 2 para o TLX1. A fusão SIL-TAL1 foi positiva em 5, a t(9;11) foi positiva
em 3, a t(10;11) em 2 e a t(11;19) em 2. A citogenética clássica foi analisada em 27
pacientes e mostrou cariótipos normais em 8, alterações numéricas em 12, e
estruturais em 8. Foram observadas as translocações t(3;13), t(17;19), t(6;11;17) e
duas t(11;14), todas definidas por citogenética molecular. A técnica de FISH mostrou
rearranjo do gene TLX3 em 4 casos e deleção em 2. Três tiveram rearranjo do gene
TCR-αβ; cinco o rearranjo SIL-TAL1 (sendo estes diferentes dos avaliados pela
molecular); e 1 rearranjo do TLX1. A amplificação do gene ABL1 foi negativa em todos
os investigados. Foram encontradas ainda as alterações estruturais i(Xq) e del7q, 11q,
12p e 14q. Em relação à alteração FLT3-ITD, 90 pacientes foram avaliados dos quais
3 positivos, dois deles sendo associado com o Hox11L2, e o terceiro sendo uma ETP.
Em relação ao Notch1, 69 pacientes foram analisados, dos quais 26 pacientes
apresentaram 22 mutações diferentes, sendo predominantemente mutação pontual.
Apesar do mal prognóstico do FLT3 na Leucemia Mieloide Aguda, todos estão vivos
no período de 5 anos após o tratamento. O impacto do Notch1 na LLA-T ainda
necessita de esclarecimentos. Em relação a citogenética, esse estudo mostra a
necessidade da citogenética molecular como auxilio diagnóstico. As translocações
t(11;14), t(17;19) e t(3;13) sugeriram mal prognóstico, já que esses pacientes recaíram
precocemente e foram a óbito.
Abstract
T-cell Acute Lymphoid Leukemia (ALL) is an aggressive hematologic malignancy that
represents 15% of childhood ALLs. Currently, specific genetic abnormalities of
importance in the diagnosis and prognosis of the disease are described. Through
cytogenetic and molecular studies it is possible to identify oncogenes involved in the
pathogenesis of the disease. The aim of this study was to determine the frequency and
impact of alterations in the FLT3 and Notch1 genes in pediatric patients with T-cell
leukemia in Pernambuco and to establish their genetic profile. In total, 129 T-ALL
patients were studied at IMIP and HUOC. Cytogenetics was evaluated by G band and
FISH, genetic profile by PCR with the panel established by the Laboratory of Molecular
Biology of IMIP, and FLT3-ITD alteration and Notch1 PEST mutation by sequencing.
Regarding the molecular panel, among the 110 analysed patients, 14 were positive for
TLX3 gene expression and 2 for TLX1. The SIL-TAL1 fusion was positive in 5 samples,
t(9;11) was positive in 3, t(10;11) in 2 and t(11;19) in 2. Classical cytogenetics was
analyzed in 27 patients and showed normal karyotypes in 8, numerical changes in 12,
and structural changes in 8. The translocations t(3;13), t(17;19), t(6;11;17) and two
t(11;14) were observed, all defined by molecular cytogenetics. FISH technique showed
TLX3 gene rearrangement in 4 cases and deletion in 2. Three had TCR-αβ gene
rearrangement; five, the SIL-TAL1 rearrangement (different from those evaluated by
molecular); and 1 rearrangement of TLX1 gene. ABL1 gene epissomal amplification
was negative in all investigated. Structural alterations i(Xq) and del7q, 11q, 12p and
14q were also found. Regarding the FLT3-ITD alteration, 90 patients were evaluated
of which 3 were positive, two of them being associated with Hox11L2, and the third
was an ETP-T-ALL. The Notch1 were analyzer in 69 patients, of which 26 presented
22 different mutations, most of them point mutations. Despite the poor prognosis of
FLT3 in Acute Myeloid Leukemia, the 3 patients in this had no events within 5 years of
post-treatment period. The impact of Notch1 on T-ALL still needs to be clarified. The
cytogenetic study showed the need for molecular cytogenetics as a diagnostic aid. The
translocations t(11;14), t(17;19) and t(3;13) suggested poor prognosis, since these
patients had early relapse of the disease and died.
