Nas últimas décadas tem havido avanços no enfrentamento da AIDS no campo biomédico, político e dos diretos das pessoas vivendo com HIV. Todavia, ainda existem desafios no controle da epidemia. No contexto brasileiro, um dos desafios refere-se ao aumento de infecções na faixa de 15 a 24 anos; ademais, de 2007 a 2022, 42,9% dos casos de HIV ocorreram entre homens por meio de relações gays ou bissexuais. Diante deste cenário e do lugar da testagem nas atuais políticas de controle da AIDS, o presente trabalho analisou a gestão de risco ao HIV entre um grupo de homens jovens que fazem sexo com homens e que realizam o teste para detecção do HIV ao menos uma vez ao ano, a partir de um estudo qualitativo. Foram realizadas entrevistas individuais com 15 homens autodefinidos como gay ou bissexual, de 18 a 24 anos, contatados durante as ações de testagem para detecção precoce do HIV promovidas por uma organização não governamental, nos municípios do Rio de Janeiro e da Baixada Fluminense. A maioria (11) dos entrevistados estava cursando o ensino superior e morava em casa própria; quatro concluíram o ensino médio e tinham menor renda. Grande parte revelou um bom conhecimento sobre os locais de acesso ao diagnóstico do HIV e os meios de prevenção do vírus da AIDS, como o preservativo e as novas tecnologias biomédicas (teste, PrEP e PEP). Na gestão dos riscos da infecção do HIV são utilizados recursos preventivos variados e a realização regular da testagem rápida, como uma prática de autovigilância sorológica. Todavia, tais práticas são influenciadas e definidas em função de fatores contextuais como: conhecimento e acesso às tecnologias de prevenção, situações de compartilhamento da identidade homossexual, redes de sociabilidade, tipos de práticas e relacionamentos sexuais e os acordos estabelecidos entre os parceiros. Os achados apontam para a necessidade de criar espaços de diálogo e aprendizagem sobre os variados recursos de prevenção disponíveis, levando em conta as visões, práticas e tipos de relacionamentos sexuais. Ademais, as estratégias de prevenção devem contemplar campanhas educativas regulares e divulgação e oferta de variadas tecnologias de prevenção, capazes de alcançar todos os segmentos sociais, bem como o enfrentamento do estigma da AIDS e da homossexualidade e as condições de vulnerabilidade ao HIV. Tais recomendações convergem com as análises críticas acerca do predomínio da biomedicalização das respostas à Aids no cenário nacional.
Abstract
In recent decades there have been advances in the fight against AIDS in the biomedical, political and rights fields of people living with HIV. However, there are still challenges in controlling the epidemic. In the Brazilian context, one of the challenges refers to the increase in infections in the 15 to 24 age group; moreover, from 2007 to 2022, 42.9% of HIV cases occurred among men through gay or bisexual relationships. Given this scenario and the place of testing in current AIDS control policies, the present study analyzed HIV risk management among a group of young men who have sex with men and who perform the test for HIV detection at least once per year, based on a qualitative study. Individual interviews were conducted with 15 men self-defined as gay or bisexual, aged 18 to 24, contacted during testing actions for early detection of HIV promoted by a non-governmental organization, in the municipalities of Rio de Janeiro and Baixada Fluminense. Most (11) of the interviewees were studying higher education and lived in their own house; four completed high school and had lower income. Most revealed good knowledge about the places to access HIV diagnosis and the means of preventing the AIDS virus, such as condoms and new biomedical technologies (test, PrEP and PEP). In managing the risks of HIV infection, various preventive resources are used, as well as regular rapid testing, as a practice of serological selfsurveillance. However, such practices are influenced and defined according to contextual factors such as: knowledge and access to prevention technologies, situations of sharing the homosexual identity, sociability networks, types of practices and sexual relationships and agreements established between partners. The findings point to the need to create spaces for dialogue and learning about the various prevention resources available, considering views, practices and types of sexual relationships. Furthermore, prevention strategies should include regular educational campaigns and the dissemination and offer of various prevention technologies, capable of reaching all social segments, as well as facing the stigma of AIDS and homosexuality and the conditions of vulnerability to HIV. Such recommendations converge with critical analyzes about the predominance of the biomedicalization of AIDS responses in the national scenario.
