Introdução. A pandemia por Coronavirus Disease 2019 (COVID-19) trouxe grandes desafios para a assistência em saúde, não sendo diferente no contexto do cuidado paliativo. Objetivo. Comparar o perfil sociodemográfico e clínico dos pacientes com câncer avançado atendidos antes e durante a pandemia por COVID-19 em uma unidade de saúde terciária especializada em cuidado paliativo. Materiais e métodos. Trata-se de uma análise de dados de pacientes atendidos antes (outubro de 2019 a março de 2020) e durante (setembro de 2020 a agosto de 2021) a pandemia por COVID-19. Foram avaliados dados sociodemográficos e clínicos. Foram empregadas análises de regressão logística, tendo como medidas de efeito o odds ratio (OR) e o intervalo de confiança (IC) de 95%. Resultados. Participaram 673 pacientes (204 no período pré-pandêmico e 469 no período pandêmico). O modelo final de regressão logística demonstrou que pacientes avaliados durante a pandemia tiveram maiores chances de ter cor de pele branca [OR: 1,66 (IC 95%: 1,15-2,39)], sítio tumoral primário localizado no trato gastrintestinal [OR: 2,95 (IC 95%: 1,55-5,62)] e em pele, ossos e tecidos moles [OR: 2,40 (IC 95%: 1,13-5,08)], ter sido submetido a tratamento radioterápico prévio [OR: 1,83 (IC 95%: 1,26-2,55)] e maior escore global da Avaliação Subjetiva Global Produzida pelo Próprio Paciente versão reduzida (ASG-PPP VR) [OR: 1,06 (IC 95%: 1,02- 1,09)]. Conclusão. Etnia, tipo de tumor primário, radioterapia prévia e risco nutricional se associaram ao câncer avançado durante a pandemia por COVID-19.
Abstract
Introduction. The Coronavirus Disease 2019 (COVID-19) pandemic brought major challenges to health care, and this is no different in the context of palliative care. Objective. To compare the sociodemographic and clinical profile of patients with advanced cancer treated before and during the COVID-19 pandemic in a tertiary health unit specialized in palliative care. Materials and methods. This is an analysis of data from patients treated before (October 2019 to March 2020) and during (September 2020 to August 2021) the COVID-19 pandemic. Sociodemographic and clinical data were evaluated. Logistic regression analyzes were used, with the odds ratio (OR) and 95% confidence interval (CI) as measures of effect. Results. 673 patients participated (204 in the pre-pandemic period and 469 in the pandemic period). The final logistic regression model demonstrated that patients evaluated during the pandemic had a greater chance of having white skin color [OR: 1.66 (95% CI: 1.15-2.39)], primary tumor site located in the gastrointestinal tract [OR: 2.95 (95% CI: 1.55-5.62)] and in skin, bones and soft tissues [OR: 2.40 (95% CI: 1.13-5.08)], have having undergone previous radiotherapy treatment [OR: 1.83 (95% CI: 1.26-2.55)] and higher global score on the Patient-Generated Subjective Global Assessment Short Form (PG-SGA SF) [OR: 1 .06 (95% CI: 1.02-1.09)]. Conclusion. Ethnicity, type of primary tumor, previous radiotherapy and nutritional risk were associated with advanced cancer during the COVID-19 pandemic.
