AVALIAÇÃO DOS PERFIS SOROLÓGICO E SÓCIO-DEMOGRÁFICO DOS RECEPTORES DE SANGUE DO HOSPITAL UNIVERSITÁRIO OSWALDO CRUZ DA UNIVERSIDADE DE PERNAMBUCO - UPE

A triagem sorológica em doadores de sangue não possibilita segurança de
100% quanto à possibilidade de transmissão de agentes infecto-contagiosos.
Apesar da obrigatoriedade da realização de testes para sífilis, hepatite B e C, HIV,
doença de Chagas, HTLV I/II e malária nas áreas endêmicas, em todas as
unidades de sangue coletadas no Brasil, não se conhece o perfil pré-transfusional
da população de receptores de sangue, em especial do receptor eventual. O
objetivo do presente trabalho foi avaliar os perfis sorológico e sócio-demográfico
dos receptores de sangue (RS) internados no Hospital Universitário Oswaldo Cruz
(HUOC) da Universidade de Pernambuco-UPE. O estudo foi realizado nos RS
(n=172) no período de fevereiro a maio de 2003, quando os pacientes foram
submetidos ao instrumento de coleta de dados. O trabalho foi aprovado pela
Comissão de Ética do Centro de Pesquisas Aggeu Magalhães-CPqAM. A
sorologia foi realizada na Fundação Hemope, e as amostras foram submetidas
aos mesmos testes utilizados para triagem dos doadores de sangue (T. pallidum,
T. cruzi, HIV, HCV, HBV e HTLV I/II). Os RS que apresentaram reação positiva
foram divididos em três grupos: grupo 1 (G1), nunca receberam sangue; grupo 2
(G2), politransfundidos; e grupo 3 (G3), receptores eventuais de sangue. O perfil
sócio-demografifico de 172 receptores de sangue do HUOC mostrou que a idade
média foi de 32,2 anos, 46,5% do sexo masculino e 53,5% feminino, 43,0%
casados, 37,0% solteiros, 20,0% outros. Quanto ao grau de escolaridade, 65,3%
tinham ensino fundamental. Apenas 22,6% residiam na cidade do Recife e 49,1%
eram oriundos de outros municípios e estados do Brasil, e 57,2% não estavam
exercendo atividade trabalhista. O estudo sorológico pré-transfusional, realizado
em 159 RS, mostrou que 62 desses indivíduos apresentaram reatividade para uma ou mais das doenças transmissíveis por sangue – 10 (13,3%) foram positivos
para T. pallidum, 4 (5,3%) para T. cruzi, 23 (30,7%) para HIV, 7 (9,3%) para HCV
e 31 (41,4%) para HBV. Nenhuma reação foi observada para HTLV I/II. A
positividade para os grupos foi: 38 (62,3%) do G1, 2 (3,3%) do G2 e 21 (34,4%)
do grupo G3. Vários RS (n=33) não tinham conhecimento do seu estado
sorológico prévio à transfusão: 19 (57,6%) do G1, 2 (100%) do G2, e 12 (63,2%)
do G3. Estes fatos apontam para a necessidade da criação de mecanismos
capazes de detectar nos receptores, antes da transfusão, a presença de agentes
patógenos transmissíveis por sangue. Isso minimizaria o risco de co-morbidade e
respaldaria o Estado e os serviços de hemoterapia quanto à presença desses
agentes nos receptores, anteriores a transfusão.
Abstract
The serological screening of blood donors does not constitute absolute
guarantee of protection against the possible transmission of infectious and
contagious agents. Despite the fact that tests for syphilis, hepatitis B and C, Aids,
Chagas’ disease and HTLV I/II, as well as malaria in endemic areas, are
compulsory in all the blood collection bags used in Brazil, the pretransfusional
serological profile of the population receiving transfusions is not known. The
present work evaluated the serological and social demographic profiles from blood
recipients (BR) from the Hospital Universitário Oswaldo Cruz (HUOC) from the
Universidade de Pernambuco-UPE. The study involved 172 BR from February to
May 2003 when the patients were submitted to the questionary. The work had the
approval of the Ethical Committee from the Centro de Pesquisas Aggeu
Magalhães-CPqAM. The serology was performed in Fundação Hemope and the
samples were submitted to the same tests used for screening the blood donors (T.
pallidum, T. cruzi, HIV, HCV, HBV e HTLV I/II). The BR that showed positive
reaction were classified into 3 groups: group 1 (G1), never received blood, group 2
(G2) multitransfused and group 3 (G3) eventual blood recipients. Concerning the
social-demographic profile of 172 BR from HUOC, the mean age was 32.2 years
old, 46.5% were male and 53.5% female, 43.0% were married, 37.0% unamarried,
20.0% others. Concerning the education level, 65.3% had just basic study. Only
22.6% lived into the city of Recife and 49.1% were from other cities and states of
Brazil . The serological study performed in 159 BR showed that 62 of these
patients presented reactivity for one or more blood-borne diseases – 10 (13.3%)
were positive to T. pallidum, 4 (5.3%) to T. cruzi, 23 (30.7%) to HIV, 7 (9.3%) to
HCV and 31 (41.4%) to HBV. No reaction was observed to HTLV-I/II. The distribution of the positivity among the groups was: 38 (62.3%) of G1, 2 (3.3%) of
G2 and 21 (34.4%) of G3. Several BR (n=33) were unaware their serological
status before their transfusion – 19 (57.6%) of G1, 2 (100%) of G2 and 12 (63.2%)
of G3. These facts point out the need to establish mechanisms capable of
detecting in the BR the presence of blood-borne pathogens prior to the
transfusion. This would minimize the risk of comorbidity and help the state health
authorities to identify the presence of such agents in the recipients before the
transfusion.
