A leishmaniose disseminada (LDi) caracteriza-se por um número elevado (≥10) de lesões cutâneas polimórficas, provavelmente devido à disseminação hemática dos parasitos, atingindo partes distintas do tegumento, podendo ou não ser acompanhada de acometimento de mucosas. É causada por espécies distintas de Leishmania e é relativamente infrequente no contexto da leishmaniose tegumentar americana. Sua resposta ao tratamento específico é alvo de controvérsia: pode responder bem ao tratamento convencional com antimoniais pentavalentes, entretanto existem casos relatados de resistência medicamentosa. O objetivo deste estudo foi a comparação do perfil genético dos parasitos isolados de pacientes com LDi e de pacientes com leishmaniose cutânea localizada (LCL), atendidos no Laboratório de Pesquisa Clínica e Vigilância em Leishmanioses, Instituto Nacional de Infectologia Evandro Chagas, Fundação Oswaldo Cruz, Rio de Janeiro, entre 1989 e 2018. Foi realizado um estudo caso-controle genotípico dos isolados derivados de fragmentos de lesões cutâneas obtidos através de biópsia antes do tratamento. Isolados de pacientes com LDi e de pacientes com LCL foram caracterizados genotipicamente utilizando primers direcionados para o minicírculo de DNA do cinetoplasto (kDNA) e para os genes codificadores da proteína do choque térmico 70 (hsp70), com sequenciamento destas regiões e comparação posterior. Todos os isolados parasitários foram caracterizados como Leishmania (Viannia) braziliensis. Houve maior variabilidade genética nos isolados de pacientes com LDi, quando comparados aos isolados dos pacientes com LCL. A análise filogenética através dos alvos selecionados não demonstrou diferenças entre os isolados de pacientes com LDi e com LCL em relação aos parâmetros clínicos (tempo de evolução, presença de comorbidades), laboratoriais por ocasião do diagnóstico (presença de amastigotas no exame histopatológico e no exame direto - escarificação e/ou impressão em lâmina; reação em cadeia da polimerase; sorologia - reação de imunofluorescência indireta e/ou ensaio imunoenzimático; intradermorreação de Montenegro), e de desfecho após o primeiro tratamento (cura ou recidiva). Houve agrupamento genotípico em um cluster de isolados de pacientes com LDi e com LCL procedentes do município do Rio de Janeiro, mostrando similaridade entre eles. Não foi possível demonstrar agrupamento distinto (clusters) entre isolados de casos com LDi e de controles com LCL através dos alvos genéticos estudados em relação aos diferentes parâmetros clínicos, laboratoriais e de desfecho. Futuros estudos com alvos distintos poderão avaliar se existem diferenças genéticas adicionais entre os parasitos isolados de casos com LDi e com LCL que possam auxiliar a compreender estas diferentes expressões clínicas da doença.
Abstract
Disseminated cutaneous leishmaniasis (DCL) is characterized by a high number (≥10) of polymorphic skin lesions, probably caused by the hematic dissemination of the parasites, reaching different parts of the skin, and which may or may not be accompanied by mucosal involvement. It is caused by distinct species of Leishmania and it is relatively infrequent in the context of American Tegumentary Leishmaniasis. Its response to specific treatment is controversial: it may respond well to conventional treatment with pentavalent antimonials, however there are reported cases of drug resistance. The aim of this study was to compare the genetic profile of the parasites isolated from patients with DCL and from patients with localized cutaneous leishmaniasis (LCL), treated at the Laboratory of Clinical Research and Surveillance in Leishmaniasis, Evandro Chagas National Institute of Infectious Diseases, Oswaldo Cruz Foundation, Rio de Janeiro, between 1989 and 2018. A genotypic case-control study of isolates derived from fragments of skin lesions obtained through biopsy before treatment was performed. Isolates from DCL patients and LCL patients were genotypically characterized using the kinetoplast DNA (kDNA) and the genes encoding the heat shock protein 70 (hsp70), with sequencing of these regions and subsequent comparison among them. All parasitic isolates were characterized as Leishmania (Viannia) braziliensis. There was greater genetic variability in isolates from DCL patients when compared to isolates from LCL patients. The phylogenetic analysis through the selected targets showed no differences between isolates from patients with DCL and LCL in relation to clinical parameters (time of evolution, presence of comorbidities), laboratory parameters at the time of diagnosis (presence of amastigotes in the histopathological examination and in the direct examination - skin smear and/or tissue printing; polymerase chain reaction; serology - indirect immunofluorescence reaction and/or immunoenzymatic assay; leishmanin skin reaction), and outcome after the first treatment (cure or relapse). There was a genotypic grouping in a cluster of isolates from patients with DCL and LCL from the city of Rio de Janeiro, showing similarity between them. It was not possible to demonstrate distinct grouping (clusters) between isolates from cases with DCL and controls with LCL using the studied genetic targets in relation to the different clinical, laboratory and outcome parameters. Future studies with different targets might be able to verify if there are additional genetic differences between the isolated parasites of cases with DCL and with LCL that may help to better understand these different clinical expressions of the disease.
