Introdução: A leishmaniose tegumentar americana (LTA) é uma doença de notificação compulsória, que apresenta um impacto significativo de saúde pública. Quando não tem a devida avaliação diagnóstica e o tratamento adequado, pode levar a sérios problemas no que se refere a imagem e aparência, em decorrência das lesões espalhadas pelo corpo, em especial na sua forma mucosa, gerando preconceito por parte da sociedade. Para enfrentar estas condições de saúde, é recomendável que o sistema de atenção à saúde promova uma atenção segura e de alta qualidade, com investimento nas tecnologias de educação em saúde por equipe multidisciplinar. Instrumentos de letramento em saúde (LS) têm sido usados para investigar as crenças, comportamentos e conhecimentos dos pacientes em relação ao cuidado em saúde. Objetivo Geral: Analisar a efetividade da estratégia educacional no letramento em saúde no contexto da leishmaniose tegumentar americana. Metodologia: Estudo prospectivo, quase-experimental, não randomizado, realizado com 51 pacientes do ambulatório do Laboratório de Pesquisa e Vigilância em Leishmanioses do Instituto Nacional de Infectologia Evandro Chagas - Fiocruz. A pesquisa foi desenvolvida em três etapas: 1ª) Adaptação e criação de um escore, para o instrumento de avalição do LS no contexto da LTA; 2ª) Desenvolvimento de uma intervenção educacional sobre a LTA; e 3ª) Coleta primária de dados. Para avaliar o letramento em saúde dos pacientes antes e após estratégia educativa, foi aplicado um instrumento de avaliação do LS no contexto clínico da LTA (Pré e Pós-teste) As associações das dimensões do LS em LTA com as variáveis categóricas, foram obtidas pelo teste chi-quadrado de Pearson ou teste exato de Fisher e para as variáveis numéricas pelo teste t ou teste de Mann-Whitney. Para verificação da normalidade das variáveis contínuas foi utilizado o teste de Shapiro-Wilk. Para comparação intra-sujeito das variáveis do pré e pós-vídeos foi utilizado teste de McNemer. Foi considerado um p valor inferior a 0,05. Resultado: Os 51 participantes tinham idade média de 56,37 anos (±17,88), a maioria era do sexo masculino, estado civil casado, branco ou pardo, com escolaridade de fundamental à ensino médio, morador da cidade do Rio de Janeiro, com uma mediana de 23 consultas realizadas (1-171) no LapClin Vigileish. A pontuação do letramento foi em média de 24,2 pontos, no pré-vídeo, e de 30,5 no pós teste (p<0,001). O percentual de participantes com letramento elevado ou adequado no pós-vídeo (100%) foi significativamente maior (p<0,001) que no pré-vídeo (60,8%). Conclusão: A estratégia educacional sobre letramento em saúde para os pacientes de LTA mostrou-se efetiva para melhoria do conhecimento dos participantes, com destaque aos aspectos epidemiológicos e do tratamento da LTA
Abstract
Introduction: American Tegumentary Leishmaniasis (ATL) is a notifiable disease that has a significant impact on public health. When it does not have the proper diagnostic evaluation and proper treatment, it can lead to serious problems about image and appearance, because of lesions spread throughout the body, especially in its mucous form, generating prejudice on the part of society. To face these health conditions, it is recommended that the health care system promote safe and high-quality care, with investment in health education technologies by a multidisciplinary team. Health literacy (HL) instruments have been used to investigate patients' beliefs, behaviors, and knowledge regarding health care. General Objective: To analyze the effectiveness of the educational strategy in health literacy in the context of American tegumentary leishmaniasis. Methodology: Prospective, quasi-experimental, non-randomized study carried out with 51 patients from the outpatient clinic of the Research and Surveillance Laboratory in Leishmaniasis of the National Institute of Infectious Diseases Evandro Chagas - Fiocruz. The research was developed in three stages: 1st) Adaptation and creation of a score for the SL assessment instrument in the context of ATL; 2nd) Development of an educational intervention on ATL; and 3rd) primary data collection. To assess the health literacy of patients before and after the educational strategy, the SL assessment instrument was applied in the clinical context of ATL (Pre and Post test) The associations of LS dimensions in LTA with categorical variables were obtained by Pearson's chi-square test or Fisher's exact test, and for numerical variables by the t test or Mann-Whitney test. To verify the normality of continuous variables, the Shapiro-Wilk test was used. For intra-subject comparison of pre- and post-video variables, the McNemer test was used. A p value of less than 0.05 was considered. Result: The 51 participants had a mean age of 56.37 years (±17.88), most were male, married, white or brown, with education from elementary to high school, living in the city of Rio de Janeiro, with a median of 23 consultations performed (1-171) at LapClin Vigileish. The literacy score averaged 24.2 points in the pre-video and 30.5 in the post-test (p<0.001). The percentage of participants with high or adequate literacy in the post-video (100%) was significantly higher (p<0.001) than in the pre-video (60.8%). Conclusion: The educational strategy on health literacy for ATL patients proved to be effective in improving the knowledge of the participants, with emphasis on the epidemiological aspects and treatment of ATL
