INTRODUÇÃO: O câncer de ovário é a neoplasia mais letal do aparelho reprodutor feminino. Carecemos de dados clínicos-patológicos que caracterizem a trajetória das pacientes brasileiras com câncer de ovário. OBJETIVO: Relacionar as características clínico-patológicas com os desfechos das pacientes com câncer epitelial de ovário em um cenário de mundo real. MATERIAL E MÉTODOS: Realizamos um estudo de série de casos, com base nos prontuários médicos das pacientes com câncer epitelial de ovário tratadas em serviço privado especializado em tratamento oncológico em Salvador/BA no período de 2005 a 2021.Foram excluídas as pacientes com dados insuficientes para refletir toda a sua trajetória desde o diagnóstico ao tratamento do câncer de ovário. RESULTADOS: 596 pacientes com CID10 C56 foram tratadas em nossa instituição no período proposto e 146 pacientes elegíveis para o estudo. A idade média ao diagnóstico foi de 60,0 anos (±12,4), 88,1% das pacientes iniciaram o tratamento em um intervalo inferior a 60 dias, 54,1% realizaram algum teste para o BRCA e a sobrevida global (SG) em 5 anos para toda a amostra foi de 64,4%. Na análise de SG em 5 anos de acordo com o ECOG houve diferença com significância estatística entre o ECOG 0 e ECOG 1 (HR 0,22, CI 95%, 0,13 - 0,72, p= 0,007). Na análise de SG em 5 anos de acordo com o estágio houve diferença com significância estatística na comparação I e II versus IV (HR 0,07, CI 95%,0,02-0,2, p = < 0,001) e III versus IV (HR 0,32, CI 95%,0,13-0,78, p=0,002). A idade média no óbito foi de 65,5 anos (±10,7). CONCLUSÃO: Avaliação retrospectiva de um único centro mais completo encontrada na literatura nacional descrevendo a trajetória da paciente com câncer de ovário. O intervalo para o início do tratamento mais curto no serviço privado sugerindo maior efetividade no tratamento de câncer de ovário em relação ao sistema público de saúde. Quase 50% das pacientes da amostra não foram submetidas a algum teste do BRCA e, portanto, oportunidades terapêuticas e de prevenção foram perdidas. Na análise univariada, estágio avançado conforme a FIGO e pacientes sintomáticas (ECOG 1) associaram-se a piores desfechos de sobrevida global
Abstract
INTRODUCTION: Ovarian cancer is the most lethal neoplasm of the female reproductive system. We lack clinicopathological data that characterize the trajectory of Brazilian patients suffering from ovarian cancer. OBJECTIVE: To relate clinicalpathological characteristics with outcomes in patients with epithelial ovarian cancer in a real-world setting. MATERIAL AND METHODS: We conducted a retrospective review of medical records from patients with epithelial ovarian cancer treated at our private cancer center in Salvador/BA from 2005 to 2021. Patients with insufficient data to reflect their entire trajectory from diagnosis to treatment of ovarian cancer were excluded. RESULTS: 596 patients with ICD56 were treated in our institution during the proposed period and 146 patients were eligible for the study. Mean age at diagnosis was 60.0 years (±12.4), 88.1% of the patients started the treatment within an interval of less than 60 days, 54,1% had any BRCA test and for the entire sample the 5-year overall survival (OS) was 64,4%. Analyzing the 5-year OS data according to the performance status (ECOG), there was a significant statistical difference between ECOG 0 and ECOG 1(HR 0,22, CI 95%, 0,13 - 0,72, p= 0,007). Looking at the 5-year OS data according to the stage, there was a significant statistical difference between I and II versus IV (HR 0,07, CI 95%,0,02 - 0,2, p = < 0,001) and III versus IV (HR 0,32, CI 95%,0,13-0,78, p=0,002). The mean age at death was 65.5 years (±10,7). CONCLUSION: This is the most complete retrospective evaluation describing the trajectory of ovarian cancer patients found in national literature. The shorter time to start treatment on a private cancer center suggests more effective results, compared to the public health system. Almost 50% patients were not submitted to a BRCA test and, therefore, therapeutic and prevention opportunities were missed. In the univariate analysis, advanced stage according to FIGO and symptomatic patients (ECOG 1) are associated with worse outcomes
