IMPLANTAÇÃO DA LINHA DE CUIDADO AO PORTADOR DE FERIDAS CRÔNICAS: UM OLHAR MULTIPROFISSIONAL

Introdução: As feridas crônicas são caracterizadas pela estagnação, lentificação e difi- culdade no processo cicatricial, o surgimento das lesões crônicas está relacionada ao comprometimento vascular, neuropatias, além de outros potenciais agravantes, incluindo a mobilidade reduzida e alterações nutricionais. Objetivo: Descrever as etapas de implantação da linha de cuidado multiprofissional ao paciente portador de feridas crônicas em uma unidade de saúde da família. Metodologia: Trata-se de um estudo descritivo, do tipo relato de experiência acerca da implantação da linha de cuidado ao portador de feridas crônicas. Resultados e Discussão: Foram divididos em etapas: Diagnóstico situacional e Identificação dos Problemas, Identificação do público alvo, Matriciamento dos Profissionais e Construção do Fluxograma multiprofissional de atendimento à pessoa portadora de ferida crônica na Unidade de Saúde da Família e Domicílio. Considerações Finais: A longitudinalidade do cuidado ao portador de feridas crônicas ocorre de fato na atenção primária em saúde, a implantação da linha de cuidados com o olhar multiprofissional favorece para que haja a transversalidade do cuidado, cujo enfoque seja o indivíduo e não somente a lesão, ultra- passando as barreiras que dificultam o processo cicatricial, evitando a fragmentação do cuidado, portanto o olhar multiprofissional é o divisor de águas para a prestação de um atendimento humanizado e integral
Abstract
Introduction: Chronic wounds are characterized by stagnation, slowness and difficulty in the healing process, the appearance of chronic lesions is related to vascular impairment, neuropathies, in addition to other potential aggravating factors, including reduced mobility and nutritional changes. Objective: To describe the implementation steps of the multidisciplinary care line for patients with chronic wounds in a family health unit. Methodology: This is a descriptive study, of the experience type about the implementation of the line of care for patients with chronic wounds. Results and Discussion: They were divided into stages: Situational Diagnosis and Identification of Problems, Identification of the target audience, Matriculation of Professionals and Construction of the multiprofessional flowchart of care for people with chronic wounds in the Family Health Unit. Final Considerations: The longitudinality of care for patients with chronic wounds actually occurs in primary health care, the implementation of the line of care with a multiprofessional look favors the transversality of care, whose focus is the individual and not just the lesion, overcoming the barriers that hinder the healing process, avoiding the fragmentation of care, therefore, the multidisciplinary approach is the watershed for providing a humanized and comprehensive care
