AVALIAÇÃO DO RISCO CARDIOVASCULAR ATRAVÉS DOS INDICADORES DE OBESIDADE E DE DISTRIBUIÇÃO DA GORDURA CORPORAL EM PORTADORES DE HIV VIRGENS DE TRATAMENTO ANTIRRETROVIRAL

O efeito da infecção pelo HIV associado a hábitos alimentares inadequados e ao aumento da prevalência de sobrepeso e obesidade em portadores de HIV tem sido responsáveis por alterações metabólicas que aumentam o risco de doenças cardiovasculares (DCV). Indicadores de obesidade associados ao perfil lipídico da dieta podem ser utilizados a fim de identificar precocemente o risco de desenvolvimento de doenças cardiovasculares antes do início da terapia antirretroviral. O objetivo do estudo foi avaliar o uso dos indicadores de adiposidade como discriminadores de risco cardiovascular (RCV) em portadores de HIV virgens de tratamento antirretroviral. Trata-se de um estudo do tipo transversal, onde foram avaliados portadores de HIV virgens de tratamento antirretroviral de março de 2012 a janeiro de 2013 atendidos no ambulatório do Instituto de Pesquisa Clínica Evandro Chagas - IPEC/FIOCRUZ. Foram coletados dados clínicos e bioquímicos, para construção do Escore de Risco de Framingham (ERF), dados antropométricos para avaliação do estado nutricional e para construção do índice de massa corporal (IMC), do índice de conicidade (IC), da razão cintura/quadril (RCQ) e da razão cintura/estatura (RCEst), e dados da ingestão alimentar. Foram avaliados 84 pacientes, a maioria do sexo masculino (81%) e com mediana de idade de 35,5 anos. De acordo com o IMC, 51,2% apresentava sobrepeso/obesidade. Apenas treze pacientes apresentaram depleção de tecido adiposo periférico, sendo três deles também com depleção de proteína somática Foram observados valores aumentados de circunferência de cintura (CC) em 45,2%, de RCEst em 67,9%, de RCQ em 15,5% e de IC em 57,1% dos pacientes. De acordo com o ERF 38,1% da amostra foi classificada com moderado/alto RCV. A ingestão de calorias não teve relação com o sobrepeso, mas a ingestão de fibras esteve inversamente associada com o colesterol total e LDL-c séricos. Pacientes com moderado/alto risco cardiovascular tiveram piores resultados de adiposidade e de perfil lipídico sérico. Observou-se que RCQ e CC, foram os indicadores de obesidade que melhor discriminam o RCV elevado, tendo a área sob a curva ROC de 0,804 (0,705-0,903) e 0,802 (0,705-0,898) respectivamente. Identificamos nesta amostra de portadores de HIV virgens de tratamento, alterações metabólicas que podem se agravar ao longo do tempo e que, se beneficiariam de intervenção dietoterápica imediata e acompanhamento nutricional.
Abstract
The effect of HIV infection associated with adverse dietary intake and the increasing prevalence of overweight and obesity in HIV-positive individuals have been responsible for metabolic changes that increase cardiovascular risk. Indicators of obesity associated with dietary lipids can be used to identify early the risk of developing cardiovascular disease before initiation of antiretroviral therapy. The objective of this study was to evaluate the use of indicators of adiposity as discriminators of cardiovascular risk (CVR) in HIV antiretroviral naïve individuals. In a cross-sectional study we evaluated HIV-infected antiretroviral naïve outpatients from March 2012 to January 2013 at the Evandro Chagas Clinical Research Institute - IPEC/FIOCRUZ. We collected clinical, biochemical, anthropometric and food intake data, to evaluate nutritional status and estimate body mass index (BMI), conicity index (CI), waist-to-hip ratio (WHR) and waist-toheight ratio (WHtR). Cardiovascular risk was estimated by the Framingham risk score (FRS). We included 84 patients, the majority male (81%) and aged 35.5 years (median). According to BMI,
51.2% were overweight/obese. Only thirteen patients had depleted peripheral adipose tissue, three of them also with depletion of somatic protein. We observed increased values of waist circumference (WC) in 45.2% of patients, WHtR in 67.9%, WHR in 15.5% and IN in 57.1% of patients. According to ERF, 38.1% of the sample was classified as moderate/high RCV. There were no association between caloric intake and overweight, but fiber intake was inversely correlated with serum total cholesterol and LDL-C. Patients with moderate/high cardiovascular risk had worse results of adiposity and serum lipid profile. WHR and WC were indicators of obesity that best discriminate the high CVR with area under the ROC curve of 0.804 (0.705 to 0.903) and 0.802 (0.705 to 0.898) respectively. We identified in this sample of HIV patients, metabolic changes that may worsen with the onset of antiretroviral treatment and therefore require prompt intervention dietary and nutritional counseling.
