Este trabalho discute a relação entre a representação e a participação social no âmbito da gestão participativa no Sistema Único de Saúde (SUS). Tem como objetivo principal analisar a representação dos usuários nos conselhos municipais de saúde. Para tanto, foi necessário revisitar os conceitos de democracia e participação para entender a sua importância para o período histórico no qual o Brasil se encontrava nos anos 1980 e para o movimento da Reforma Sanitária. Nesta perspectiva, foi abordada a institucionalização da gestão participativa, bem como foi feita uma revisão bibliográfica acerca dos estudos que versam sobre os conselhos de saúde com a finalidade de compreender melhor os limites e as possibilidades destas instâncias. Em seguida, foram discutidas a interlocução entre os conceitos de representação e participação, conceitos estes centrais para o funcionamento da gestão participativa, bem como a influência da representação na dinâmica dos conselhos de saúde. Também foi problematizado o conceito de usuário do SUS, pois além de ser o ator central desta dissertação não existe nenhuma definição legal sobre ele. Com o intuito de analisar a representação dos usuários nos conselhos municipais de saúde foi feito um mapeamento das entidades que atuam representando este segmento em todos os 5.564 conselhos municipais de saúde do Brasil. Estas entidades foram analisadas a partir das variáveis Brasil, regiões e estados e os resultados foram discutidos à luz das questões conceituais advindas da interlocução entre participação/representação. Por fim, esta dissertação pretende contribuir para o debate da gestão participativa ao disparar debates que envolvam uma maior articulação entre os conceitos de representação e participação e os atravessamentos possibilitados por eles.
Abstract
This paper discusses the relationship between the representation and social participation within the context of the participative administration in the Brazilian Unified Health System (SUS). The main goal of this work is to analyze the representation of the users in the local health care counsels. It was thus necessary to revisit the concepts of democracy and participation in order to understand its importance to the historical context in which Brazil found itself during 1980, and also to understand the movement of the Sanitary Reform. This paper takes said context into account and presents an analysis of the institutionalization of the participative administration as well as a revision of the literature pertaining to the studies that address health care counsels. The purpose here is to better understand the limits and possibilities of these configurations. A discussion of the interlocution between the concepts of representation and participation is presented afterwards. These concepts are fundamental towards the functioning of the participative administration, and they also dictate the influence of representation in the dynamics of health care counsels. The concept of user that is presented by the Brazilian Unified Health System (SUS) was also problematized, since despite being the central author of this dissertation, there is no legal definition of the user. A mapping of the entities that act as representatives of the users was made with the purpose of analyzing their representation in the 5.564 local health care counsels in Brazil. These entities were analyzed through the following variables: Brazil, regions and states. The results were discussed through the prism of the conceptual issues that arose from the interlocution between participation/representation. The purpose of this paper is, thus, to contribute towards the debate of participative administration by sparking debates that involve a larger articulation between the concepts of representation and participation and the intersections that they facilitate.
