No Município do Rio de Janeiro, a atenção primária em saúde (APS) passou por um
intenso processo de expansão pelo aumento da cobertura da Estratégia Saúde da Família (ESF),
ocasião que propiciou a contratação de farmacêuticos para atuar nas unidades básicas de saúde.
Este profissional contribui no fortalecimento da APS, por meio da prestação Serviços
Farmacêuticos (SEFAR). Este estudo objetivou analisar as contribuições do curso Serviços
Farmacêuticos na Atenção Primária em Saúde (SFAPS) para a prática profissional e gestão do
trabalho. Distintas estratégias metodológicas envolveram análise documental (AD) de diversos
documentos internos ao curso e a realização de grupo focal (GF) com atores-chave; além da
aplicação de questionário autorrespondido dirigido aos egressos do curso formados entre 2011
e 2019. As duas primeiras estratégias (AD e GF) permitiram descrever os antecedentes, além
de características sobre o curso. O GF e o questionário responderam sobre as contribuições,
fragilidades e fortalezas da formação, além do perfil dos egressos. Face à complementaridade
entre os métodos, os resultados foram analisados transversalmente e os dados qualitativos
passaram por análise temática de conteúdo. O curso contou com 13 ofertas entre 2011-2019,
com 328 alunos formados e baixa taxa de evasão (5,7%). Sua estrutura curricular foi
padronizada ao longo dos anos, operacionalizada a partir de metodologias ativas de
aprendizagem e avaliação apoiada em projetos de intervenção. De maneira geral, os
respondentes apresentaram elevados graus de concordância (graus 4 e 5) sobre as diversas
afirmativas das contribuições do curso SFAPS. As questões técnicas gerenciais e assistenciais,
especialmente a gestão de estoques e a dispensação qualificada de medicamentos foram
sinalizadas como maiores, enquanto a implementação de trabalhos de final, a capacidade de
colocar em prática temas discutidos, a realização de visita domiciliar e o gerenciamento
adequado de resíduos dos serviços mostraram menores graus de concordância. Ampliação do
conhecimento e a carga horária de 80 horas foram reconhecidos, na perspectiva dos egressos,
como principais fortalezas e fragilidades do curso, respetivamente. A baixa governabilidade
dos farmacêuticos na tomada de decisão gerencial pode interferir sobre as ações assistenciais
nas unidades da APS. O curso mostrou-se como estratégia de qualificação do cuidado em saúde,
na perspectiva de prestação de serviços farmacêuticos aos usuários e à equipe de saúde.
Abstract
Primary Health Care (PHC) at Rio de Janeiro County went through an intense expansion
because of Family Health Strategy (FHS) coverage increase, which propitiated pharmacists
hiring to work at primary health centers. This professional contributes to PHC strengthening by
Pharmaceutical Services (PhSer) offer. This study aimed to analyze Pharmaceutical Services in
Primary Health Care (PSPHC) course’s contributions to professional practice and work
management. Different methodological approach included documental analysis (DA) of several
internal course documents and a focus group (FG) realization with key actors and self-answered
questionnaire application addressed to the course egresses graduated between 2011 to 2019.
The two first approaches (DA and FG) allowed describing background, in addition to the course
characteristics. FC and questionnaire answered about contributions, weaknesses, and strengths
of training, in addition to egresses profile characterization. Due to methods complementarity,
results were transversally analyzed, and qualitative data were submitted to thematic content
analysis. The course counted on 13 offers between 2011 and 2019, with 328 graduated students
and low dropout rates (5,7%). It’s curricular structure has been standardized over the years,
operationalized from active learning and assessment methodologies supported by intervention
projects. In general, respondents showed high levels of agreement (4 and 5 levels) on different
statements about PSPHC course. Technical management and assistance issues, especially
inventory management and qualified to dispense of medicines were signaled as the greatest,
while the implementation of end-of-course papers, the ability to put into practice discussed
topics, the performance of home visits, and the adequate management of service waste showed
lower degrees of agreement. The knowledge expansion and the 80-hour workload were
recognized, from egresses perspective, as the course’s main strengths and weaknesses,
respectively. Low governability of pharmacists in managerial decision-making can interfere
with care actions in PHC units. The course proved to be a strategy to qualify health care from
the perspective of providing pharmaceutical services to users and to the health team.
