Desde o início da epidemia de COVID-19, diante da elevada transmissibilidade da infecção e objetivando achatar a curva epidêmica foram adotadas medidas de distanciamento social e restrições da mobilidade social. Mesmo no atual cenário de disponibilidade de vacinas, ainda se faz necessário responder qual é o impacto da mobilidade na transmissibilidade da infecção pelo SARS-CoV-2. Esse estudo teve como objetivo identificar a relação entre a mobilidade social e a trasmissibilidade da infecção pelo SARS-CoV-2 em Recife durante a fase ascendente da primeira onda da epidemia em 2020. Trata-se de um estudo ecológico misto, a unidade espacial é o bairro (94) de Recife e a temporal, o dia (62). A fonte de dados de casos de COVID-19 foram os boletins epidemiológicos divulgados diariamente pela Secretaria Municipal de Saúde de Recife, esses dados foram utilizados para o cálculo da medida de transmissibilidade da infecção, o número reprodutivo dependente do tempo (Rt), calculado pelo método de Wallinga & Teunis. O distanciamento social foi mensurado pelo Índice de Isolamento Social (IIS) da base de dados da empresa de tecnologia de informação Inloco. Foram construídos modelos de regressão lineares generalizados (GLM) com distribuição binomial negativa para 4 lags temporais (5, 7, 10 e 14), considerando os dias que o IIS e o Rt foram medidos. O conjunto mínimo de variáveis de ajuste foi obtido a partir do gráfico acíclico direcionado (DAG), incluindo no modelo indicadores socioeconômicos extraídos do censo demográfico 2010 do IBGE e indicadores ambientais do projeto WorldClim. As análises foram realizadas utilizando os softwares R e QGIS. Nossos resultados sugerem que existe associação inversa entre o distanciamento social e a medida de transmissibilidade da infecção pelo SARS-CoV-2 nos bairros de Recife, considerando a proporção de população raça/cor preta e parda, proporção de domicílios com renda média per capita de até ½ salário-mínimo e densidade populacional como variáveis de ajuste. Ressalta-se que foi possível observar que o IIS, utilizado como medida de distanciamento social, foi maior nos finais de semana e nos bairros de melhores condições socioeconômicas. Enquanto o Rt apresentou valores mais elevados em bairros de melhores condições socioeconômicas nas primeiras semanas, porém com posterior aumento da medida na periferia da cidade, não sendo possível observar um padrão de distribuição espacial. Nossos resultados foram semelhantes a estudos anteriores e o presente estudo contribuiu para indicar a necessidade de enfrentar as dificuldades metodológicas.
Abstract
Since the beginning of the COVID-19 epidemic, given the high transmissibility of the infection and aiming to flatten the epidemic curve, measures of social distance and restrictions on social mobility were adopted. Even in the current scenario of availability of vaccines, it is still necessary to answer what is the impact of mobility on the transmission of infection by SARS-CoV-2. This study aimed to identify the relationship between social mobility and the transmissibility of SARS-CoV-2 infection in Recife during the ascending phase of the first wave of the epidemic in 2020. This is an ecological study, the spatial unit is the district (94) of Recife and the time unit, the day (62). The source of data on COVID-19 cases were the epidemiological reports published daily by the Municipal Health Department of Recife, these data were used to calculate the measure of infection transmissibility, the time-dependent reproductive number (Rt), calculated by Wallinga & Teunis method. Social mobility was measured by the Social Isolation Index (IIS) from the information technology company Inloco's database. Generalized linear regression models (GLM) with negative binomial distribution were built for 4 temporal lags (5, 7, 10 and 14), considering the days IIS and Rt were measured. The minimum set of adjustment variables was obtained from the directed acyclic graph (DAG), including in the model socioeconomic indicators extracted from the 2010 IBGE census and environmental indicators from the WorldClim project. Analyzes were performed using R and QGIS software. Our results showed that there is an inverse association between social distancing and the measure of transmissibility of SARS-CoV-2 infection in Recife districts, considering the proportion of black and brown race/color population, proportion of households with average per capita income of up to ½ minimum wage and population density as adjustment variables. It is noteworthy that it was possible to observe that the IIS, used as a measure of social distancing, was higher on weekends and in districts with better socioeconomic conditions. While Rt showed higher values in districts with better socioeconomic conditions in the first weeks, but with subsequent increase in the measure in the outskirts of the city, it was not possible to observe a pattern of spatial distribution. Our results were similar to previous studies and the present study contributed to indicate the need to confront methodological difficulties.
