O câncer de mama é o mais incidente e a principal causa de morte entre as mulheres no mundo, sendo considerado uma preocupação de saúde pública global. O tratamento para esta neoplasia é agressivo e a presença de efeitos adversos é comum, sendo a fadiga oncológica um dos principais relatados. A etiologia da fadiga oncológica é multifatorial, estando relacionada à própria biologia tumoral e aos tratamentos oncológicos isolados ou combinados, além de sofrer a influência de outras condições como dor, sofrimento emocional, anemia, distúrbios no sono, desequilíbrio nutricional, diminuição da funcionalidade e comorbidades. A associação da fadiga com esses fatores, assim como suas interrelações, não está ainda bem elucidada. Sendo assim, esta tese teve por objetivo analisar os fatores associados à fadiga oncológica e as suas interrelações na linha de base de uma coorte de mulheres com câncer de mama incidente e, posteriormente, a associação desses fatores com a variação da fadiga ao longo do tratamento neoadjuvante. Primeiramente foi realizado um estudo seccional na linha de base da coorte, constituída por 724 mulheres com câncer de mama matriculadas no Instituto Nacional do Câncer e, por meio de uma análise de caminhos, analisou-se as interrelações entre a fadiga oncológica e os fatores associados identificados na literatura. Sintomas depressivos, insônia, índice de massa corporal, idade, estado civil, nível de atividade física e força de preensão palmar estiveram associados de forma direta à fadiga oncológica antes do início do tratamento. Posteriormente, foi realizado um estudo de coorte prospectivo com 427 mulheres submetidas à quimioterapia neoadjuvante. A avaliação da fadiga e demais fatores associados foi realizada no início, entre ciclos e ao término do tratamento. O método estatístico utilizado foi o GEE (Generalized Estimating Equations). A média do escore de fadiga diminuiu até o final do segundo esquema, representando a piora do sintoma. O aumento na idade, a presença de sobrepeso e obesidade, o relato de insônia, a presença de sintomas depressivos e o relato de dor pioraram significativamente este sintoma. Esses resultados possibilitam estratificar e tratar não só a fadiga oncológica como os fatores associados à mesma em mulheres com câncer de mama.
Abstract
Breast cancer is the most incident and the leading cause of death among women in the world, being considered a global public health concern. The treatment of this neoplasm is aggressive, and the presence of adverse effects is common, with cancer related fatigue being one of the most reported. The etiology of cancer related fatigue is multifactorial, being related to the tumor biology itself and to isolated or combined cancer treatments, and being influenced in addition by other conditions such as pain, emotional distress, anemia, sleep disorders, nutritional imbalance, decreased functionality and comorbidities. The association of fatigue with these factors and their interrelations are still not well understood. Therefore, this thesis aimed to analyze the factors associated with cancer fatigue in the literature and their interrelationships at the baseline of a cohort of women with incident breast cancer and, subsequently, the association of these factors with the variation in fatigue throughout neoadjuvant treatment. Firstly, a cross-sectional study was carried out at the baseline of the cohort of women with breast cancer enrolled at the National Cancer Institute through a path analysis of 724 women. It was possible to analyze the interrelationships between fatigue and its associated factors. Depressive symptoms, insomnia, body mass index, age, marital status, level of physical activity and hand grip strength were directly associated with cancer fatigue before the start of treatment. Subsequently, a prospective cohort study was conducted with 427 women submitted to neoadjuvant chemotherapy. The assessment of fatigue and its associated factors was performed at the beginning, between cycles and at the end of the treatment. The statistical method used was GEE (Generalized Estimating Equations). The average fatigue score decreased until the end of the second regimen, representing the worsening of the symptom. The increase in age, the presence of overweight and obesity, the report of insomnia, the presence of depressive symptoms and the report of pain significantly worsened this symptom. These results make it possible to stratify and treat not only cancer related fatigue but the associated factors in women with breast cancer.
