O consumo excessivo de sódio e insuficiente de potássio, quando relacionados entre si, pode ocasionar um risco aumentado para doenças cardiovasculares, e, entender os aspectos da dieta que envolvem esses nutrientes, por meio da análise de padrão alimentar, pode ser uma ferramenta essencial na prevenção desses desfechos em gestantes. Este estudo teve como objetivo estimar a associação entre padrões alimentares e a razão sódio e potássio da dieta em gestantes atendidas na rede primária de atenção à saúde nos municípios de Macaé (Rio de Janeiro) e Pinhais (Paraná). Trata-se de um estudo transversal, realizado a partir de um recorte de um estudo multicêntrico, que buscou avaliar a deficiência de iodo, sódio e potássio no público materno-infantil nas cinco macrorregiões do Brasil. Este estudo, portanto, utilizou dados das cidades de Macaé (RJ) e Pinhais (PR). Participaram do estudo um total de 490 gestantes atendidas na rede primária de atenção à saúde desses municípios. Dados sociodemográficos das participantes foram coletados a partir de um questionário semiestruturado, enquanto os dados de saúde e alimentação foram coletados por meio de autorrelatos e cartão de saúde da gestante e um recordatório de 24 horas, respectivamente. A análise dos padrões alimentares se deu por análise fatorial exploratória e a razão sódio e potássio foi estimada a partir da dieta das gestantes. Um modelo de regressão logística foi desenvolvido para testar a associação entre potenciais variáveis de confusão (vaiáveis sociodemográficas, de saúde e estilo de vida) e a razão sódio e potássio; e um outro modelo de regressão logística testou a associação entre os padrões alimentares e a variável desfecho. A média de idade das participantes do estudo foi de 27 anos (DP = 6,1), e mais da metade delas possuíam ensino médio (60,5%) Não foi observada associação entre a razão sódio e potássio e as características sociodemográficas e de saúde estudadas (p>0,05), com exceção da ingestão de energia, no qual as gestantes com razão sódio e potássio maior que 1 (61%) apresentaram ingestão energética total maior que aquelas com a razão menor ou igual a 1 (39%) (p<0,05). Dessa forma, somente a ingestão energética foi considerada nos modelos múltiplos. Os resultados dos modelos de regressão logística bivariados e dos modelos múltiplos ajustados para o consumo de energia total mostraram associações positivas entre os padrões “café da manhã” (leites e derivados, gorduras e óleos e pães) e “lanches” (carnes processadas, sal, molhos e temperos industrializados e lanches salgados) e associações negativas entre os padrões alimentares “prudente” (carnes de aves e seus produtos, sucos de fruta e/ou vegetais naturais) e “in natura” (vegetais e seus produtos, frutas, cafés e chás, raízes e tubérculos) e a razão sódio e potássio da dieta acima de 1. Considerando os modelos ajustados pela ingestão energética, as gestantes que aderiram ao padrão “café da manhã” e “lanches”, tiveram 62% (OR 1,62; IC95% 1,30-2,02) e 152% (OR 2,52; IC95% 1,83-3,47) mais chances de apresentarem a razão sódio e potássio dietético maior que 1, respectivamente. Enquanto que as mulheres que aderiram aos demais padrões, “prudente” e “in natura”, tiveram menor chance de apresentarem uma razão sódio e potássio maior que 1 (OR 0,64; IC95% 0,52-0,79 e OR 0,45; IC95% 0,35-0,58, respectivamente) Apenas o padrão “tradicional brasileiro” não apresentou associação estatisticamente significativa com a razão sódio e potássio da dieta tanto no modelo bruto quanto no modelo ajustado pela ingestão energética total. Dos 5 padrões alimentares identificados neste estudo, os padrões “café da manhã” e “lanches” mostraram associações positivas e estatisticamente significativas com a razão sódio e potássio da dieta acima de 1, enquanto os padrões “prudente” e “in natura” mostraram associações negativas e estatisticamente significativas com a razão sódio e potássio da dieta acima de 1.
Abstract
Excessive consumption of sodium and insufficient potassium, when related to each other, can cause an increased risk for cardiovascular diseases, and understanding the aspects of the diet that involve these nutrients, through dietary pattern analysis, can be an essential tool in prevention of these outcomes in pregnant women. This study aimed to estimate the association between dietary patterns and the sodium and potassium ratio of the diet in pregnant women treated in the primary health care network in the municipalities of Macaé (Rio de Janeiro) and Pinhais (Paraná). This is a cross-sectional study, carried out based on an excerpt from a multicenter study, which sought to evaluate iodine, sodium and potassium deficiency in maternal and child populations in the five macro-regions of Brazil. This study, therefore, used data from the cities of Macaé (RJ) and Pinhais (PR). A total of 490 pregnant women attended in the primary health care network in these municipalities participated in the study. Participants' sociodemographic data were collected from a semi-structured questionnaire, while health and dietary data were collected through self-reports and the pregnant woman's health card and a 24- hour recall, respectively. The analysis of dietary patterns was carried out using exploratory factor analysis and the sodium and potassium ratio was estimated based on the pregnant women's diet. A logistic regression model was developed to test the association between potential confounding variables (sociodemographic, health and lifestyle variables) and the sodium and potassium ratio; and another logistic regression model tested the association between dietary patterns and the outcome variable. The average age of study participants was 27 years old (SD = 6.1), and more than half of them had secondary education (60.5%) No association was observed between the sodium and potassium ratio and the sociodemographic and health characteristics studied (p>0.05), with the exception of energy intake, in which pregnant women with a sodium and potassium ratio greater than 1 (61%) presented total energy intake greater than those with a ratio less than or equal to 1 (39%) (p<0.05). Therefore, only energy intake was considered in the multiple models. The results of bivariate logistic regression models and multiple models adjusted for total energy consumption showed positive associations between the “breakfast” patterns (milk and derivatives, fats and oils, and bread) and “snacks” (processed meats, salt , industrialized sauces and seasonings and savory snacks) and negative associations between “prudent” eating patterns (poultry meat and its products, fruit juices and/or natural vegetables) and “in natura” (vegetables and their products, fruits, coffee and teas, roots and tubers) and the dietary sodium and potassium ratio above 1. Considering the models adjusted for energy intake, pregnant women who adhered to the “breakfast” and “snacks” pattern had 62% (OR 1. 62; 95%CI 1.30-2.02) and 152% (OR 2.52; 95%CI 1.83-3.47) more likely to have a dietary sodium and potassium ratio greater than 1, respectively. While women who adhered to the other standards, “prudent” and “in natura”, were less likely to have a sodium and potassium ratio greater than 1 (OR 0.64; 95%CI 0.52-0.79 and OR 0 .45; 95%CI 0.35-0.58, respectively) Only the “traditional Brazilian” pattern did not show a statistically significant association with the dietary sodium and potassium ratio in both the crude model and the model adjusted for total energy intake. Of the 5 dietary patterns identified in this study, the “breakfast” and “snacks” patterns showed positive and statistically significant associations with the dietary sodium and potassium ratio above 1, while the “prudent” and “in natura” patterns showed associations negative and statistically significant with the dietary sodium and potassium ratio above 1.
